Transitioning from pediatric to adult clinic is referred to as Health care transition.\u00a0 Health care transition is the process of a moving from pediatric to adult clinic with or without transferring to a new provider. It involves planning, transfer, and integration into adult-centered health care.\u00a0<\/span>\u00a0<\/span><\/p>\n

The goals of health care transition are:\u00a0<\/span>\u00a0<\/span><\/p>\n

1) To improve the ability of youth and young adults to manage their own health care needs and effectively use health services,<\/span>\u00a0<\/span><\/p>\n

2) To provide a process in pediatric and adult health care practices prepare for this transition.<\/span>\u00a0<\/span><\/p>\n
