To improve colorectal cancer screening, breast cancer screening, cervical cancer screening, pneumococcal vaccination and influenza vaccination in the UNC Primary Care Improvement Collaborative practices.

• Preventive services reminders now going out to patients in 70 clinics
• 190,437 unique patients impacted by outreach services in FY18
• About 5,200 patients reached monthly by Birthday outreach for preventive services
• Four employees working in population health outreach
