In order to promote patient safety, health care professionals need to experience psychological safety themselves. A just culture \u2013 one that recognizes that people work in systems, and that system failures are to blame for errors rather than individuals \u2013 is essential to supporting greater reliability within organizations.<\/p>\n<p>UNC Health has the <u><a href=\"https:\/\/unchcs.sharepoint.com\/sites\/MCRisk\/SitePages\/SAFE.aspx\" target=\"_blank\" rel=\"noopener\">SAFE reporting system<\/a><\/u> to identify errors in patient care. All employees are encouraged to submit SAFE reports for errors and near misses.
