Admissions Checklist
We are excited about your arrival! In the weeks before you begin your medical school career at UNC, there will be several tasks that we will ask accepted applicants to complete.

Immunization Records Update
Due: June 15, 2023
You will need to submit your immunizations to “Viewpoint Screening.” Please keep an eye out for an email about that soon. If you are not a former UNC undergraduate student, you must also upload your immunization documents to ConnectCarolina for Campus Health Services.

Transcripts
Due: July 1, 2023
You have two options for sending transcripts, “snail” mail, or electronically.
Hard Copy Transcripts
- Hard copies of final official transcripts should be sent to:
- UNC School of Medicine, Office of Admissions, 321 S. Columbia Street, Suite 1001 Bondurant Hall, CB# 9500, Chapel Hill, NC, 27599-9500
Electronic Transcripts
- If your school uses an electronic service to send transcripts, please send transcripts to: admissions@med.unc.edu
- If your school uses the National Clearinghouse service to send transcripts, our account information is:
- National Clearinghouse: UNC School of Medicine – Office of Admissions – 00297400sm – PDF. Make sure the correct school on our campus is selected!
It is your responsibility to ensure that all transcripts are received in our office by July 1, 2023. Failure to provide the required transcripts confirming successful completion of the UNC SOM prerequisites, program coursework, and degrees earned, could result in one of the following:
- Revocation of your acceptance, or
- A stop on your account, which will prevent you from receiving financial aid.
Complete Required Health and Safety Trainings
Due: June 15, 2023
- Under the “Clinic” heading, complete Clinic Bloodborne Pathogens Course (4140) and Tuberculosis Awareness and Infection Control Course (4130).
- Under the “Other Non-EHS Training” heading, complete HIPAA Requirements.
- Please upload your training certificates for each of the following to SLATE:
- Clinic Bloodborne Pathogens Course (4140),
- Tuberculosis Awareness and Infection Control Course (4130),
- and HIPAA Requirements and Health & Safety Agreement form.
