Before Placing a Referral<\/h3>\n

Before placing a referral, please review our \u201cwho we care for\u201d page<\/a> which outlines the conditions we are able to best care for.<\/p>\n<\/som-cell>\n

Use a faxed referral to refer a patient<\/h3>\n

Please fax a completed referral form along with relevant medical records (e.g., clinic notes and test results) to the Cardiology Referral Center 984-974-2990. Upon receipt, the staff will review the request and identify a provider. Next, our scheduling team will call to set up an appointment. Should you need to check to see if an appointment has been made, please call 984-974-7244.<\/p>\n

Use EPIC CareLink to refer patients and receive results<\/h3>\n

If you are a non-UNC physician or mid-level provider you can now use the UNC CareLink portal to directly place referrals, access information about your patients (including scheduled appointments, clinical notes, and more) and send secure messages to UNC cardiology regarding your patients\u2019 care.<\/p>\n

\n<\/u><\/strong>Please have your physician (either primary care or specialist) fax a completed clinic referral form (Nephrology Referral Form<\/a>), along with relevant medical records (e.g., clinic notes and test results) to 984-974-2985.\u00a0Please do mention \u201cReferral for Hypertension clinic\u201d.<\/strong>\u00a0Upon receipt, the staff will review the request and identify whether you need to be seen in a multidisciplinary clinic with cardiology or with our Hypertension expert alone. Next, our scheduling team will call to set up an appointment. Finally, we will fax your physician a copy of the appointment time and mail you information about your appointment.
