DESCRIPTION:Dr. Sartor is a gastroenterologist and mucosal immunologist with a long-standing interest in mechanisms by which commensal microbiota induce chronic intestinal inflammation vs. mucosal homeostasis. He specializes in inflammatory bowel diseases (IBD) and directs the UNC Multidisciplinary IBD Center. Dr. Sartor has been a faculty member at UNC for his entire academic career. His research is focused on developing and applying rodent models of chronic\, immune-mediated intestinal inflammation relevant to IBD and performing clinically relevant translational studies of IBD patients. Dr. Sartor investigates host genetically programmed immune responses to luminal commensal microbial products using gnotobiotic rodents and patient-derived samples and studies the influence of diet and environmental factors on intestinal microbiota composition and function.

DESCRIPTION:Dr. Andrew Greganti will review the clinical course of a patient who initially presented with a polymyalgia rheumatica syndrome and whose prolonged illness was complicated by a cardiomyopathy of unclear pathogenesis. \nDr. Anil Gehi is an electrophysiologist in the UNC Center for Heart & Vascular Care specializing in pacemaker and defibrillator implantation\, device extraction\, catheter ablation for SVT\, VT\, and atrial fibrillation. He is involved with research studies on atrial fibrillation with respect to symptom relief and catheter ablation. Whether by performing a catheter ablation\, implanting a device\, or simply providing reassurance that worrisome symptoms are benign\, he enjoys helping patients through their illness. He also works to improve the lives of patients with cardiac dysrhythmia by advancing systems of care and developing more precise ablation techniques. Finally\, he helps future physicians acquire knowledge and skills for success in medicine\, cardiology\, and electrophysiology. \nDr. Greganti specializes in the care of elderly patients and the differential diagnosis of complex disease processes. He is Vice Chair of the Department of Medicine with primary responsibility for grand rounds.

SUMMARY:Medicine Grand Rounds\, Timothy Ives and Maryam Jowza\, "Opioid Use and Misuse"
DESCRIPTION:The US is in the midst of an opioid overdose epidemic. Overdose deaths involving prescription opioids have quadrupled since 1999\, and so have sales of these prescription drugs. Efforts to address the opioid epidemic have focused mainly on reducing non-medical opiate use. Overprescribing of opiate agents has led to a sharp increase in the prevalence of opioid addiction\, which is also associated with a rise in overdose deaths and heroin use. A multifaceted public health approach is required to effectively reduce opioid-related morbidity and mortality. The scope of options to manage patients with chronic pain issues\, along with strategies to mitigate the risks associated with opioid agents\, will be discussed during this Medicine Grand Rounds. \n \nDr. Timothy Ives is the Director of Pain Management Service\, Division of General Medicine and Clinical Epidemiology\, Professor of Medicine and Professor of Pharmacy. His research interests are in the area of inflammation and chronic pain management\, with neuropathic pain in particular. \nDr. Maryam Jowza is Assistant Professor of Anesthesiology. She treats a wide range of pain conditions from acute post-surgical pain to chronic musculoskeletal disorders\, cancer-related pain and neuropathic conditions. She also has a special interest in treating pain in children.

DESCRIPTION:Dr. Anne Cappola is a Professor of Medicine in the Division of Endocrinology\, Diabetes\, and Metabolism at the University of Pennsylvania. She is Director of the Clinical and Translational Research Center and Director of Clinical Research for the Department of Medicine. She is an Associate Editor for the Journal of the American Medical Association. \nDr. Cappola’s research focuses on the hormonal alterations that occur with aging and the clinical impact of these changes. She is funded by the National Institute on Aging. Current projects include studies of the indications for treatment of subclinical thyroid dysfunction in older men and women; the role of androgens in the health of older women; hormonal treatments for the frailty syndrome; and metabolic changes in men and women who sustain a hip fracture. \nHer lecture will focus on subclinical hypothyroidism in the elderly\, combination treatment with T4+T3\, thyroid hormone extracts\, and information about nutraceuticals — “natural” treatments.

SUMMARY:Medicine Grand Rounds\, Stephan Moll\, "Thrombophilia Evaluation: Choosing Wisely"
DESCRIPTION:Dr. Stephan Moll’s expertise is in thrombosis and anticoagulation. In this presentation\, he will set forth clinically important facts about the pathophysiology\, prevalence and thrombotic risk of inherited and acquired thrombophilias.  In addition\, he will describe which patient and family members to test for thrombophilia\, what tests to obtain\, and at what time to test. The implications of a thrombophilia on an individual’s health also will be presented\, in addition to detailed information about how thrombophilia tests are done in the laboratory and what parameters (acute thrombosis\, presence of anticoagulant) may influence the test results. \nDr. Moll is Professor of Medicine in the UNC Hemophilia and Thrombosis Center. His clinical interest is coagulation and classical hematology\, with a particular focus on thrombosis and anticoagulation.  Dr. Moll’s research interests include clinical trials on new anticoagulants\, better use of established anticoagulants\, antiphospholipid antibody syndrome and postthrombotic syndrome. He takes a special interest in clinical-medical education of patients\, the public\, and health care professionals. He is the cofounder of the thrombosis and anticoagulation education website Clot Connect – at www.clotconnect.org.

DESCRIPTION:Because of the prevalence\, seriousness\, and treatability of sepsis\, UNC Hospitals launched Code Sepsis\, an initiative that aims to implement reliable screening for early detection of sepsis\, educate multidisciplinary teams in evidence-based therapies\, and standardize tools and treatment bundles for our specific patient populations. \nMost patients and their families have never heard of sepsis\, a condition that kills more people every year than breast cancer\, prostate cancer\, and AIDS combined. Studies show that when health-care providers promptly recognize sepsis and treat it by immediately administering antibiotics and fluids\, survivorship increases by 50%. Although sepsis most often occurs in the elderly\, the young\, and those with chronic diseases such as cancer or liver disease\, sepsis can affect perfectly healthy people of all ages. Every year\, in fact\, more than 1.6 million Americans develop sepsis during hospitalization\, and nearly a quarter-million of those die from sepsis-related complications. A majority of sepsis survivors who are discharged are readmitted to the hospital within 30 days of leaving. \nThe important information delivered through this lecture will help our health-care providers better understand sepsis\, the current progress of the initiative\, and access to the tools and training materials that are being tested or implemented as part of the initiative. \nTina Schade Willis\, MD\, is Professor of Pediatric Critical Care Medicine\, Associate CMO for Quality at UNC Hospitals\, and Director of the UNC Institute for Healthcare Quality Improvement. Her academic focus includes system designs that reduce variation and cycle times leading to improved patient care\, efficiency\, and satisfaction. She is an expert in high performance team training\, performance improvement study design\, analysis\, and management.

DESCRIPTION:The eosinophilic gastrointestinal diseases (EGIDs) represent a spectrum of conditions where high numbers of eosinophils infiltrate the GI mucosa and cause pathologic changes and clinical symptoms.  They are classified by where the eosinophilic infiltration predominates. Eosinophilic esophagitis (EoE) is the most common and well-studied\, and now represents a substantial cause of upper GI morbidity. In contrast\, eosinophilic gastritis (EG)\, gastroenteritis (EGE)\, and colitis (EC) are rare and relatively little is known about them. This talk will review current understanding of EGIDs\, including their diagnosis\, epidemiology\, treatment\, outcomes\, and challenges\, and will highlight research advances. \nDr. Evan Dellon is Associate Professor of Medicine and Epidemiology in the Division of Gastroenterology and Hepatology. He also directs the UNC Center for Esophageal Diseases and Swallowing. His research interests are in esophageal diseases\, and eosinophilic esophagitis in particular. He has multiple active protocols studying the epidemiology\, diagnosis\, pathogenesis\, and treatment of EoE. He collaborates with allergists\, pediatric gastroenterologists\, pathologists\, and epidemiologists at UNC\, throughout the US\, and around the world. \n“When you can look back at pathology samples from the 70’s\, 80’s\, and 90’s\, you just don’t see the level of inflammation in the esophagus\, with eosinophils. People didn’t see the findings in the esophagus when they did endoscopies in the 1980’s. Something has clearly changed. The incidence is increasing. It’s becoming more and more common.”

DESCRIPTION:Dr. Hladik is Chief of the Division of Nephrology and Hypertension and the Doc J Thurston Distinguished Professor of Medicine. He also serves as Medical Director of the Acute Dialysis Unit. His clinical interests include the evaluation of acute and chronic kidney disease\, the diagnosis and management of complex electrolyte disorders\, and geriatric nephrology. As Director of the Nephrology Training Program he works to improve education in nephrology spanning from foundational training in kidney physiology and nephrology during medical school\, residency\, and fellowship\, through continuing education for practicing nephrologists. He currently is Education Director for Maintenance of Certification for the American Society of Nephrology. In this role\, he serves as co-editor for the Nephrology Self-Assessment Program.
