DESCRIPTION:Dr. Rubin and Dr. Kim will discuss the approach to primary hyperparathyroidism using cases to highlight diagnosis in the office\, hand off from medicine to surgery\, the surgical approach and considerations of atypical patient presentations and follow-up. \n \nDr. Janet Rubin practices Endocrinology with a specialty in Osteoporosis and metabolic bone disease\, and is responsible for bone density testing. Dr. Rubin continues her decades-long research into bone remodeling\, funded by the NIH. Dr. Rubin is also active in DOM leadership as the Vice-Chair for Research. Her interests are in junior faculty research development\, and promotion of research throughout the Department. \nDr. Larry Kim is a recognized expert in surgical diseases of the endocrine glands.  He has written on a variety of topics related to these conditions and has served as Program Chair of the annual meeting of the American Association of Endocrine Surgeons. He is particularly recognized in the use of office-based ultrasound for endocrine diseases and teaches ultrasound at national and international courses.

SUMMARY:Emily Chang and Andrej Lyshchik\, "Current and Emerging Applications for Contrast-Enhanced Ultrasound
DESCRIPTION:Drs. Chang and Lyshchik will discuss the emerging technology of contrast-enhanced ultrasound. Dr. Chang will present examples of cases when contrast-enhanced ultrasound can enhance clinical care. Dr. Lyshchik will describe the evolution of this technology\, current indications\, future applications and challenges faced by the community. \nDr. Chang is a nephrologist who began investigating various ultrasound techniques for patients with kidney disease during her fellowship. She is currently the PI on a study investigating the use of contrast-enhanced ultrasound (CEUS) in patients with kidney disease and indeterminate or suspicious kidney lesions. She has spoken at national American Institute of Ultrasound in Medicine (AIUM) conferences on the nephrologist’s perspective on contrast-enhanced ultrasound. \nDr. Lyshchik is a radiologist from Thomas Jefferson University with a proven history of excellence in the field of contrast-enhanced ultrasound. As Vice-Chair of American College of Radiology CEUS Liver Imaging Reporting and Data System (LI-RADS) working group and steering committee member\, his work was instrumental to the recent FDA approval of a contrast agent for liver imaging. In addition\, he is a Vice-Chair of the AIUM Contrast-Enhanced Ultrasound Community\, which provides a program for their annual convention. He has delivered multiple talks on CEUS and interventional ultrasound at major radiological and ultrasound meetings in the US. He has recently created the first observership in the United States so that radiologists can obtain training and exposure from an institution with experience in this sector. He is a national leader in the field.

SUMMARY:Medicine Grand Rounds\, Ned Sharpless\, "The Surprising Relationship Between Cancer and Aging\, and What This Means for an Internist"
DESCRIPTION:Dr. Ned Sharpless is the Director of the Lineberger Comprehensive Cancer Center and a practicing oncologist. He runs a 15-person basic science group that employs genetically engineered mice to study cancer and aging. His research has focused on how normal cells age and undergo malignant conversion. Dr. Sharpless discovered a fundamental biological link between cancer and aging\, and has used this knowledge to develop murine models and human diagnostic tests of molecular age. \nWork from his group has described novel regulators of metastasis\, identified pharmacological approaches to protecting stem cells in vivo\, and characterized circular RNAs\, a novel form of non-coding RNA. He has authored more than 130 original reports\, reviews and book chapters\, and is an inventor of 14 filed or issued patents.

DESCRIPTION:Dr. Nielsen’s clinical practice is focused in urologic oncology—in particular the treatment of bladder\, prostate and kidney cancer. He is a member of the integrated Multidisciplinary Genitourinary Oncology Group at the Lineberger Comprehensive Cancer Center\, where he serves as Director of the Division of Urologic Oncology. Dr. Nielsen is committed to providing compassionate\, individualized\, patient-centered care\, and has been selected by his peers to be named among the Best Doctors in America in Urology since 2013.  Alongside his clinical practice\, Dr. Nielsen serves in leadership roles in Patient Safety and Quality Improvement activities locally as well as on the national level. \nDr. Nielsen’s research in medical decision making\, cancer care quality and treatment outcomes has been funded by the National Institutes of Health\, the Agency for Healthcare Research and Quality and The American Cancer Society\, and nationally recognized by the Rising Stars in Urology Research Award. DESCRIPTION:Dr. Berkowitz’s academic career has centered on his role as residency program director at UNC\, a position he held until 2014. Currently the Department of Medicine Vice Chair for Education\, he has also served as president of the Association of Program Directors in Internal Medicine\, chair of the test writing committee of the American College of Physicians In-training examination\, and chair of the American Board of Internal Medicine Council. \nIn this Grand Rounds\, Dr. Berkowitz will examine how physicians spend their time in patient care and how it can be redirected to improve patient care and work satisfaction.

SUMMARY:Medicine Grand Rounds\, John Vavalle and Thomas Caranasos "Leading the Charge: A New Era in the Treatment of Valvular Heart Disease"
DESCRIPTION:In 2014\, Dr. Vavalle and Dr. Caranasos successfully performed the first TAVR procedure here at UNC. The TAVR program recently completed its 100th procedure; it is quickly growing into one of the busiest programs in North Carolina. Transcatheter aortic valve replacement (TAVR) is a new technology that provides a treatment option for many patients with aortic valve stenosis who previously had no other treatment option\, or were at high risk for standard open surgical valve replacement due to advanced age\, frailty\, or co-morbid illness.  \n\nDr. Vavalle also leads the UNC Heart Valve Clinic where a multi-disciplinary team of cardiac surgeons\, cardiologists\, and advanced practice providers evaluate patients with valvular heart disease to determine the best treatment options available. Dr. Caranasos serves as a co-director of the TAVR Program along with Dr. Vavalle and Dr. Michael Yeung.  He is director of Adult Cardiac Surgery.

SUMMARY:Medicine Grand Rounds\, Christopher Sayed "DRESS Syndrome and Related Drug-Induced Dermatitis"
DESCRIPTION:Drug reaction with eosinophilia and systemic symptoms (DRESS) is a rare\, potentially life-threatening\, drug-induced hypersensitivity reaction that includes skin eruption\, hematologic abnormalities (eosinophilia\, atypical lymphocytosis)\, lymphadenopathy\, and internal organ involvement. DRESS is characterized by a long latency (two to eight weeks) between drug exposure and disease onset\, a prolonged course with frequent relapses despite the discontinuation of the culprit drug\, and frequent association with the reactivation of latent human herpesvirus infections. DRESS will be reviewed in this lecture\, along with related drug-induced dermatitis. \nDr. Sayed practices Adult Dermatology and Dermatologic Surgery. He has a special interest in treating Hidradenitis Suppurativa and follicular disorders\, along with skin complications of inflammatory bowel disease.

DESCRIPTION:This lecture is presented by Chief Residents Emily Sturkie\, Evan Shelby and Cole Sims. They will discuss major changes to our residency program in the past year\, including regionalization of our medicine teams\, daily admissions\, and our new ambulatory curriculum for interns. They will explain why they made these changes\, how they did it\, and results on patient care and education of our residents which includes exposure to subspecialty ambulatory experiences.  \n\nDr. Sturkie is UNC Hospitals Chief. After residency\, she plans to pursue a career in hospital medicine. The other UNC Hospitals Chief\, Dr. Shelby\, is working toward to a career in gastroenterology with a focus on medical education. Dr. Sims is UNC Ambulatory Medicine Chief. He aspires to work in geriatrics and general medicine after his chief year.
