DESCRIPTION:Dr. Runge is executive vice president for Medical Affairs at the University of Michigan\, Dean of the Medical School\, and CEO of Michigan Medicine. Prior to joining the University of Michigan in March 2015\, he was executive dean and chair of the UNC Department of Medicine\, where he was instrumental in guiding the academic and clinical leadership of the School of Medicine and the UNC Health Care System.  He was also principal investigator and director of the North Carolina Translational and Clinical Sciences (NC TraCS) Institute at UNC-Chapel Hill.  \nDr. Runge has been a physician-scientist for his entire career\, combining basic and translational research with the care of patients with cardiovascular diseases and education. He is the author of over 200 publications in the field and holds five patents for novel approaches to health care. \nThis is the inaugural Laurence E. Earley lecture\, honoring Dr. Earley (1931-2012)\, an internationally known nephrologist who retired from UNC in 2000.

DESCRIPTION:Dr. Blythe was an expert in salt and water homeostasis. This lecture will use a case-based approach to review the overall principles of salt and water homeostasis\, and will describe new paradigms in long term salt balance\, the storage of salt in the tissues and the neural control of salt appetite and thirst. Future advances in salt and water homeostasis will enhance the therapy of hypertension and shock\, as well as disorders of water metabolism. \nDr. Mark Zeidel is Herrman L. Blumgart Professor of Medicine at Harvard Medical School\, and Chair and Physician-in-Chief at Beth Israel Deaconess Medical Center in Boston. His research has been recognized by the American Physiological Society\, the American Society for Clinical Investigation and the Association of American Physicians. His research has received continuous funding by the NIH for more than 30 years. \nEvery one to three years\, the Division of Nephrology and Kidney Center invites a lecturer to honor the memory of Dr. William Blythe\, II. Dr. Blythe\, the first Division Chief for Nephrology and Hypertension at UNC\, was an outstanding physician-scientist and teacher who inspired many of us to pursue nephrology. He was also a gifted man of letters.

SUMMARY:Medicine Grand Rounds\, Steven Russell "The Bionic Pancreas"
DESCRIPTION:The bionic pancreas system proved better than either conventional or sensor-augmented insulin pump therapy at managing blood sugar levels in patients with type 1 diabetes living at home\, with no restrictions\, over 11 days. “For study participants living at home without limitations on their activity and diet\, the bionic pancreas successfully reduced average blood glucose\, while at the same time decreasing the risk of hypoglycemia\,” says Steven Russell. “This system requires no information other than the patient’s body weight to start\, so it will require much less time and effort by health care providers to initiate treatment. And since no carbohydrate counting is required\, it significantly reduces the burden on patients associated with diabetes management.”  \nSince 2006 Steven Russell has collaborated on this project with Dr. Edward Damiano\, a Biomedical Engineer at Boston University. The trials are scheduled to begin in the fourth quarter of 2018.

SUMMARY:Medicine Grand Rounds\, Sascha Tuchman and Koyal Jain\, "Amyloidosis\, Cyanide\, Myeloma and Ethanol: Conceptualizing Renal Diseases Caused by Monoclonal Gammopathies"
DESCRIPTION:Monoclonal gammopathies are a group of plasma cell or B-cell disorders including true cancers such as multiple myeloma or “sort-of cancers” such as light chain amyloidosis. These disorders in many cases cause no injury to a person (i.e.\, monoclonal gammopathy of undetermined significance [MGUS]) but when they do cause injury\, the pattern of injury can be quite varied and often includes the kidneys. Monoclonal gammopathy of renal significance (MGRS) is a recently described category of nephrotroxic monoclonal gammopathies that arise in the absence of an overt hematologic malignancy. Monoclonal gammopathies overall have been increasingly recognized as a cause of renal failure. Over 660\,000 Americans have kidney failure\, and early recognition of the etiology of kidney disease can prevent progression to end-stage kidney disease through aggressive interventions\, such as chemotherapy in proven cases of multiple myeloma or MGRS. The management of these patients requires close collaboration between nephrologists and oncologists. In this presentation\, we will be focusing on the wide spectrum of diseases associated with nephrotoxic plasma cell disorders\, with a focus on how to conceptualize them clinically. \nSascha Tuchman is a UNC Comprehensive Cancer Center member and Associate Professor of Medicine. Dr. Tuchman’s research has a special focus on amyloidosis and multiple myeloma.  \nKoyal Jain is an Assistant Professor of Medicine in the Nephrology division. She specializes in acute kidney injury and chronic kidney disease\, dialysis management\, high blood pressure management\, abnormalities of electrolytes like sodium\, potassium\, calcium\, and glomerular diseases.

SUMMARY:Medicine Grand Rounds\, Saira Sheikh “BAFFling New Lupus Therapies – Looking to the Future After Decades of Failed Clinical Trials”
DESCRIPTION:Systemic lupus erythematosus (SLE) is a chronic\, debilitating autoimmune disease with heterogeneous manifestations ranging from scarring skin rash or arthritis to severe\, organ-threatening involvement of the kidneys\, heart\, lungs or central nervous system. SLE is characterized by the presence of autoantibodies against nuclear antigens\, which are thought to be caused by dysregulation of the immune system. Development of new therapies is critical due to unmet needs associated with SLE\, including inadequate disease control\, poor health-related quality of life\, medication toxicities and increased morbidity and mortality. \nOne challenge in caring for patients with SLE is a lack of approved and efficacious medications. Randomized controlled trials evaluating novel biologic immune modulators in SLE have been largely unsuccessful at achieving the primary endpoints needed for FDA approval. Efforts to develop therapies against specific molecular targets in SLE has resulted in the identification of BAFF (B-cell activating factor) inhibitors to treat the disease. This talk will provide a summary of that journey and a brief overview of what’s on the horizon for SLE therapies. \nDr. Saira Sheikh is an Assistant Professor of Medicine\, Director of Clinical Trials Program at the UNC Thurston Arthritis Research Center and Director of the UNC Rheumatology Lupus Clinic. Dr. Sheikh’s work has been focused on building a Collaborative Lupus Clinical & Research Program that directly impacts the care of patients. She is working on a national level to develop programs that address racial disparities in lupus clinical trials.

SUMMARY:Medicine Grand Rounds\, Todd Baron and Ian Grimm "Recent Advances in Therapeutic Endoscopy"
DESCRIPTION:Once deemed primarily diagnostic\, endoscopy has quickly transcended to a minimally invasive intervention and therapeutic tool. This continuous evolution of the spectrum of endoscopy represents an active area of ongoing research and clinical care with tremendous biomedical potential. This review will focus on latest developments in the field of therapeutic endoscopy \nIan Grimm\, MD\, has been practicing advanced therapeutic gastrointestinal endoscopy for nearly 30 years and has had a longstanding interest in the non-surgical removal of complex polyps. In 2016\, he undertook a sabbatical to study endoscopic submucosal dissection and per-oral endoscopic myotomy in Japan\, where these techniques were initially developed. Dr. Grimm has served as director of the GI Procedures unit at UNC Memorial Hospital since 2000. \nTodd Baron\, MD\, performs endoscopic treatment of complex gastrointestinal disorders\, particularly management of pancreaticobiliary obstruction through the use of endoscopic retrograde cholangiopancreatography and endoscopic ultrasound. He also conducts extensive clinical research in endoscopic management of diseases throughout the gastrointestinal tract. Dr. Baron is Director of Advanced Therapeutic Endoscopy at UNC. \nWatch the video

This session will describe approaches that institutions and individual practices can take reduce burnout and improve the joy\, purpose and meaning in work for health professionals.  \nChristine Sinsky\, MD\, is the vice president of professional satisfaction at the American Medical Association. She is also a practicing internist in Dubuque\, Iowa\, and the Chair of the American Board of Internal Medicine Foundation. SUMMARY:Medicine Grand Rounds\, Katya Rubinow "Can Sex Steroids Help Elucidate the Utility of HDL As an Index of Cardiovascular Risk?"
DESCRIPTION:In epidemiologic studies\, sex steroid deficiency predicts cardiovascular risk in both men and women\, but the utility of hormone replacement therapy for reducing cardiovascular risk remains uncertain. Exogenous sex steroids regulate plasma concentrations of high-density lipoprotein cholesterol (HDL-C)\, but little is known about sex steroid-mediated effects on other metrics of HDL composition and function. Understanding the roles of sex steroids in regulating the function and composition of HDL particles may lend new insight into the cardioprotective potential of hormone replacement therapy and\, further\, may help elucidate the utility of HDL-based metrics for predicting cardiovascular risk on an individual basis.  \nKatya Rubinow\, MD\, is a board-certified physician at the Endocrinology/Diabetes Clinic at Harborview and a University of Washington assistant professor of Medicine and Metabolism\, Endocrinology and Nutrition. Dr. Rubinow cares for patients with diabetes\, thyroid disease and a broad spectrum of other endocrine disorders. She strives to help patients feel supported and understood and to provide information that will enable them to be powerful advocates for their own health. \nWatch the video

O’Gara\, MD\, is the Director of Strategic Planning for the Cardiovascular Division at Brigham and Women’s Hospital\, and the Watkins Family Distinguished Chair in Cardiology and Professor of Medicine at Harvard Medical School. His clinical activities are focused on patients with valvular heart disease\, other structural heart diseases and aortic diseases\, although he also maintains a large practice of general cardiology. Since 2001\, he has been named annually among the Top Doctors in Boston and the U.S. \nDr. O’Gara is presenting the annual Ernest and Hazel Craige lecture which honors Dr. Ernest Craige\, the first chief of the UNC Division of Cardiology\, who served from 1952 until 1978. \nWatch the video
