Under the supervision of an attending gastroenterologist, fellows perform <\/span><span class=\"NormalTextRun SCXW147247454 BCX8\">consultations<\/span> <span class=\"NormalTextRun SCXW147247454 BCX8\">and<\/span> <span class=\"NormalTextRun SCXW147247454 BCX8\">provide<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> follow-up care for inpatients at UNC Medical Center experiencing a variety of GI luminal disorders, including upper &amp; lower GI bleeding, inflammatory bowel disease, functional bowel &amp; motility disorders, dysphagia, abdominal pain, and diarrhea. For continuity of care, <\/span><span class=\"NormalTextRun SCXW147247454 BCX8\">fellows perform<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> endoscopic procedures for the<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> patients<\/span> <span class=\"NormalTextRun SCXW147247454 BCX8\">they see in consultation<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> and provide post-discharge outpatient care for patients who are not already <\/span><span class=\"NormalTextRun SCXW147247454 BCX8\">established<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> with <\/span><span class=\"NormalTextRun SCXW147247454 BCX8\">another gastroenterology<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> attending, advanced practice provider, or fellow. This service\u202fincludes two fellows<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> to split consultation responsibilities and maximize continuity of care with endoscopic procedures.<\/span> <span class=\"NormalTextRun SCXW147247454 BCX8\">In <\/span><span class=\"NormalTextRun SCXW147247454 BCX8\">addition, often<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> one or more rotating residents and<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\">\/or<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> medical students <\/span><span class=\"NormalTextRun SCXW147247454 BCX8\">are <\/span><span class=\"NormalTextRun SCXW147247454 BCX8\">on the team, which <\/span><span class=\"NormalTextRun SCXW147247454 BCX8\">provides<\/span><span class=\"NormalTextRun SCXW147247454 BCX8\"> extensive opportunities for fellows to hone their cognitive and bedside teaching skills.<\/span><\/span><span class=\"EOP SCXW147247454 BCX8\" data-ccp-props=\"{&quot;134233117&quot;:true,&quot;134233118&quot;:true}\">\u00a0<\/span><\/p>\n<h2>Hepatology Consultation Service<\/h2>\n<p><span class=\"TextRun SCXW105817818 BCX8\" lang=\"EN-US\" xml:lang=\"EN-US\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW105817818 BCX8\">On the Hepatology Consultation Service, fellows develop <\/span><span class=\"NormalTextRun SCXW105817818 BCX8\">expertise<\/span><span class=\"NormalTextRun SCXW105817818 BCX8\"> in the diagnosis and management of hospitalized patients with liver diseases. Like the Luminal Consult Service, fellows perform consultations on hospitalized patients at UNC Medical Center. During this rotation, fellows see patients with disorders such as acute liver failure, decompensated liver diseases, variceal bleeding, hepatic encephalopathy, spontaneous bacterial peritonitis, post-liver transplant complications, abnormal liver enzymes\/function tests, and various other conditions. <\/span><span class=\"NormalTextRun SCXW105817818 BCX8\">Fellows will also gain primary experience with liver transplant evaluation and listing. <\/span><span class=\"NormalTextRun CommentStart CommentHighlightPipeRest CommentHighlightRest SCXW105817818 BCX8\">For continuity of care, fellows are encouraged to <\/span><span class=\"NormalTextRun CommentHighlightRest SCXW105817818 BCX8\">perform <\/span><span class=\"NormalTextRun CommentHighlightRest SCXW105817818 BCX8\">endoscopic<\/span><span class=\"NormalTextRun CommentHighlightRest SCXW105817818 BCX8\"> procedures on thes<\/span><span class=\"NormalTextRun CommentHighlightRest SCXW105817818 BCX8\">e<\/span><span class=\"NormalTextRun CommentHighlightRest SCXW105817818 BCX8\"> patients, <\/span><span class=\"NormalTextRun CommentHighlightRest SCXW105817818 BCX8\">e<\/span><span class=\"NormalTextRun CommentHighlightRest SCXW105817818 BCX8\">specially in emergency cases. <\/span><span class=\"NormalTextRun CommentHighlightPipeRest SCXW105817818 BCX8\">Patients seen in consultation who are not previously seen by a specific GI faculty membe<\/span><span class=\"NormalTextRun SCXW105817818 BCX8\">r or <\/span><span class=\"NormalTextRun SCXW105817818 BCX8\">fellow <\/span><span class=\"NormalTextRun SCXW105817818 BCX8\">at UN<\/span><span class=\"NormalTextRun SCXW105817818 BCX8\">C are <\/span><span class=\"NormalTextRun SCXW105817818 BCX8\">subsequently<\/span><span class=\"NormalTextRun SCXW105817818 BCX8\"> managed by the fellow<\/span> <span class=\"NormalTextRun SCXW105817818 BCX8\">in cont<\/span><span class=\"NormalTextRun SCXW105817818 BCX8\">i<\/span><span class=\"NormalTextRun SCXW105817818 BCX8\">nuity clinic should follow-up become necessary. data-contrast=\"auto\"><span class=\"NormalTextRun SCXW92190894 BCX8\">On the<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\"> Biliary\/A<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">dvanced Endoscopy Consultation Service, fellows gain exposure to advanced therapeutic endoscopy with a focus on pancreaticobiliary procedures. <\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">There are typically two advanced endoscopy attendings performing procedures in the Main endoscopy unit daily. Procedures include ER<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">CP, diagn<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">ostic EUS, therapeutic EUS (including gastrojejunostomy, <\/span><span class=\"NormalTextRun SpellingErrorV2Themed SCXW92190894 BCX8\">hepaticogastrostomy<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">, EDGE, gallbladder drainage), endoscopic stenting, and placement of percutaneous jejunostomy tube. <\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">In addition, there are opportunities to gain exposure to advanced endoscopic resection and third-space endoscopy (POEM). <\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">The service is supported by an <\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">experienced<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\"> nurse practitioner who <\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">splits<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\"> consultations with the fellow <\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">and helps to coordinate outside procedure requests.<\/span> <span class=\"NormalTextRun SCXW92190894 BCX8\">T<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">he 4<\/span><\/span><span class=\"TextRun SCXW92190894 BCX8\" lang=\"EN-US\" xml:lang=\"EN-US\" data-contrast=\"auto\"><span class=\"NormalTextRun Superscript SCXW92190894 BCX8\" data-fontsize=\"12\">th<\/span><\/span><span class=\"TextRun SCXW92190894 BCX8\" lang=\"EN-US\" xml:lang=\"EN-US\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW92190894 BCX8\">-year Advanced Endoscopy fellow helps to support the service as well<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">.\u00a0<\/span><span class=\"NormalTextRun SCXW92190894 BCX8\">\u00a0<\/span><\/span><span class=\"EOP SCXW92190894 BCX8\" data-ccp-props=\"{&quot;134233117&quot;:true,&quot;134233118&quot;:true}\">\u00a0<\/span><\/p>\n<h2>Ambulatory Rotation<\/h2>\n<p><span class=\"TextRun SCXW23644612 BCX8\" lang=\"EN-US\" xml:lang=\"EN-US\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW23644612 BCX8\">The Ambulatory Rotation<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\">s<\/span> <span class=\"NormalTextRun SCXW23644612 BCX8\">are<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\"> one of the unique features of the UNC GI &amp; Hepatology Fellowship Program.<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\"> During the Ambulatory Rotation<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\">s<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\">, fellows spend half-days rotating through our <\/span><span class=\"NormalTextRun SCXW23644612 BCX8\">numerous<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\"> sub-specialty programs\u202fin our centers of excellence, working alongside expert faculty in inflammatory bowel disease, esophageal diseases, functional bowel disorders, motility disorders,<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\"> and<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\"> hepatology<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\">. Within these clinics, fellows get experience seeing tertiary and quaternary care referrals while working with world-renowned experts, such as Hans <\/span><span class=\"NormalTextRun SpellingErrorV2Themed SCXW23644612 BCX8\">Herfarth<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\">\u202f(IBD)\u202fand Nick <\/span><span class=\"NormalTextRun SCXW23644612 BCX8\">Shaheen<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\"> (esophageal disease). <\/span><span class=\"NormalTextRun SCXW23644612 BCX8\">During ambulatory time, fellows also have exposure to\u202fthe work up of\u202furgent\u202foutpatient referrals\u202fin a weekly urgent clinic<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\"> as well as weekly continuity clinic<\/span><span class=\"NormalTextRun SCXW23644612 BCX8\">.<\/span><\/span><span class=\"EOP SCXW23644612 BCX8\" data-ccp-props=\"{&quot;134233117&quot;:true,&quot;134233118&quot;:true}\">\u00a0<\/span><\/p>\n<h2>Ambulatory Hepatology Rotation<\/h2>\n<p>The Ambulatory Hepatology rotation provides fellows with exposure to outpatient management of\u00a0pre- and\u00a0post-transplant\u00a0patients, diagnostic work up of abnormal LFTs,\u00a0management of liver masses, and\u00a0outpatient management of chronic\u00a0and end stage\u00a0liver disease.\u00a0Fellows on this rotation also have exposure to outpatient endoscopic procedures in patients with liver disease, including endoscopic procedures for varices.\u00a0Faculty are experts in\u00a0general and transplant\u00a0hepatology, viral\u00a0hepatology, fatty liver disease, autoimmune liver disease and liver cancer.\u00a0Fellows on this rotation also participate in liver conferences including liver pathology conference, multidisciplinary\u00a0pancreatobiliary\u00a0conference and transplant selection\u00a0conference.<\/p>\n<h2>Endoscopy fellows receive dedicated training in both the cognitive and procedural aspects of endoscopy. <\/span><span class=\"NormalTextRun SCXW219424769 BCX8\">Under faculty supervision, fellows learn the skills needed to perform diagnostic, screening, and surveillance upper endoscopy and colonoscopy, as well as therapeutic aspects of the endoscopy, such as\u202fmucosal biopsy, polypectomy, hemostasis, <\/span><span class=\"NormalTextRun SCXW219424769 BCX8\">variceal banding, <\/span><span class=\"NormalTextRun SCXW219424769 BCX8\">stricture dilation, and gastrostomy tube placement.<\/span><span
