Our Same Day Clinic is located at Eastowne on the 5th floor.<\/li>\n<\/ul>\n<p style=\"text-align: right\"><a href=\"https:\/\/www.med.unc.edu\/medicine\/eastowne\/\">Learn more about UNC Eastowne.\u00a0<\/a><\/p>\n<h4>Additional services offered to patients established with UNC Internal Medicine<\/h4>\n<ul>\n<li><a href=\"https:\/\/www.med.unc.edu\/medicine\/im\/patient-care\/internal-medicine-travel-clinic\/\"><strong>Travel Clinic<\/strong><\/a>\u00a0\u2013 Immunizations, other medications and traveler\u2019s advice for patients planning travel abroad.<\/li>\n<li><strong>Medication Assisted Treatment (MAT)<\/strong> \u2013 Substance use care for patients of our clinic, which includes screening and diagnosis for substance use disorder, medication treatment for substance use with buprenorphine\/naxolone (Suboxone<sup>TM<\/sup>) or naltrexone (Vivitrol<sup>TM<\/sup>), support from specialty addiction medicine services and counseling, and support from social work and a licensed clinical addiction specialist (LCAS).<\/li>\n<li><strong>Counseling<\/strong> \u2013 Short term goal focused for some types of mood and health issues. If this is the right treatment, your doctor can make a referral. If other treatment is indicated, your doctor can provide information about possible resources.<\/li>\n<li><strong>Anticoagulation\/Warfarin Clinic<\/strong> \u2013 Monitoring and safe prescribing of anticoagulants (e.g. warfarin, Eliquis\u00ae, Xarelto\u00ae, Pradaxa\u00ae) to limit the risk of bleeding events while lowering your risk of clot or stroke.<\/li>\n<li><strong>Diabetes Program<\/strong>\u00a0\u2013 Visits with Advance Practice Providers who specialize in diabetes treatment and medication adjustments. Longer one-on-one visits for diabetes education are available.<\/li>\n<li><strong>Dietitian\/Nutrition visits<\/strong>\u00a0\u2013\u00a0Medical Nutrition Therapy for patients\u00a0with chronic conditions. This includes patients with diabetes, hypertension, high cholesterol, GI disorders and those who want to lose or gain weight.<\/li>\n<li><strong>Chronic Pain Clinic<\/strong>\u00a0\u2013 Medication pain management provided by Advance Practice Providers for patients actively seeing a primary care provider in our clinic.<\/li>\n<li><strong>Heart Failure\u00a0<\/strong>\u2013 Visits with a Clinical Pharmacist Practitioner to adjust medicines under close monitoring to guideline recommended doses to reduce the risk of heart failure exacerbations, hospitalizations, and cardiovascular death.<\/li>\n<li><strong>Medication Management<\/strong> \u2013 Visits with a Clinical Pharmacist Practitioner for a complete medication review. This visit focuses on making sure you are taking the right medications, at the right dose, at the right time. The pharmacist will ensure that medications are monitored for safety. They will provide education to patients and caregivers. They will help with medication organization to make it easier to take your medicines as prescribed.<\/li>\n<li><strong>Hospital Follow-Up<\/strong> \u2013 A visit within 2 weeks of being discharged from the hospital. This visit is to follow-up on the recommendations from the hospital and lower your risk of needing to be readmitted to the hospital. This visit is with a team of providers (physician, pharmacist, social work) who will provide expertise on your medical diagnosis, medications, and social needs.<\/li>\n<li><a href=\"https:\/\/www.med.unc.edu\/medicine\/im\/patient-care\/clinical-services\/medicare-patients\/\"><strong>Medicare Annual Wellness Visits<\/strong><\/a> \u2013 Designed to improve your overall health care by providing a more detailed look at your health risks based on family history, health behaviors and more.<\/li>\n<li><strong><a class=\"internal-link\" title=\"\" href=\"https:\/\/www.med.unc.edu\/medicine\/im\/patient-care\/clinical-services\/medicare-patients\/\" target=\"_self\" rel=\"noopener noreferrer\">Chronic Care Management (CCM) Program<\/a><\/strong> \u2013 Patients with chronic medical conditions are provided individual and personal care to better manage their health.<\/li>\n<\/ul>\n<h4>Pharmacy and Laboratory Services<\/h4>\n<a class=\"btn btn-primary btn-lg  oscitas-bootstrap-container\" target=\"_self\" href=\"https:\/\/www.med.unc.edu\/medicine\/im\/patient-care\/clinical-services\/pharmacy-and-lab-services\/\">Learn More<\/a>\n<h4>Financial Assistance<\/h4>\n<a class=\"btn btn-primary btn-lg  oscitas-bootstrap-container\" target=\"_self\" href=\"https:\/\/www.med.unc.edu\/medicine\/im\/patient-care\/clinical-services\/financial-assistance\/\">Learn More<\/a>\n","protected":false},"excerpt":{"rendered":"<p>We offer the following Clinic Services for patients 18 years or older Adult Primary Care Clinics \u2013 Focusing on disease prevention, health promotion, disease management, diagnostic evaluation and acute care.
