Upon receipt, the staff will review the request and identify the appropriate provider. Choose \u201cAmbulatory Referral to Nephrology\u201d and select \u201cUNC Nephrology at Eastowne\u201d.<\/p>\n<h3>Use EPIC CareLink to refer a patient<\/h3>\n<p><strong>If you are a non-UNC physician or mid-level provider,<\/strong> you can use the UNC CareLink portal to directly place referrals, and access information about your patients (including scheduled appointments, clinical notes, and more).<\/p>\n<p>Find more information on \u202f<a href=\"https:\/\/www.med.unc.edu\/medicine\/gi\/wp-content\/uploads\/sites\/1267\/2021\/12\/PDF-MedCtr-UNC-CareLink-Information.pdf\">UNC Health<\/a>\u00a0and\u202f<a href=\"http:\/\/www.unccarelink.org\/\"><span data-contrast=\"none\">enroll for UNC CareLink here<\/span><\/a>.<\/p>\n<h2>After a Referral Has Been Placed<\/h2>\n<p>Upon receipt of the referral, whether placed through EPIC, EPIC CareLink, or fax, our scheduling team will review the request and identify the appropriate provider(s). If needed, the scheduling team will consult with our medical director or a clinic nephrologist. On occasion, additional information may be requested.<\/p>\n<hr \/>\n<div><i>NB: Severe\/life threatening electrolytes\/kidney injury\/other urgent\/emergent issues should be evaluated in emergent care locations.
