In addition to providing quick guidance to requesting providers, E-Consults serve to decrease clinic wait time for new patient referrals and allow faculty to focus on preferred diagnoses that align with areas of specific interest and expertise in clinic.<\/p>\n<p>E-Consults may take up to five business days for a response.<\/p>\n<p>E-consults (electronic consults) are available to any UNC Health physician or advanced practice provider. At present, E-consults are not available to providers outside the UNC Health System.<\/p>\n<p>E-consults are not for logistical questions or for questions that can be easily answered by consulting UpToDate or published clinical guidelines. <strong>E-consults should not be used for urgent, emergent or acute care questions.<\/strong> Additionally, E-consults should not be used to ask follow-up questions for patients who have been previously seen by providers in our practice.<\/p>\n<h3>Conditions Appropriate for Nephrology E-Consult<\/h3>\n<ul>\n<li>eGFR 45&#8211;60 ml\/min\/1.73m<sup>2<\/sup> and\/or UACR 30-299 mg\/g (<strong>with<\/strong> comorbid DM). Review <a href=\"https:\/\/www.med.unc.edu\/medicine\/nephrology-hypertension\/wp-content\/uploads\/sites\/1307\/2024\/11\/CKD20Management-combined.pdf\">CKD management goals<\/a> and<a href=\"https:\/\/www.med.unc.edu\/medicine\/nephrology-hypertension\/wp-content\/uploads\/sites\/1307\/2024\/11\/CKD-Care-Pathway_workup.pdf\"> CKD carepathways.\u00a0<\/a><\/li>\n<li>eGFR 30&#8211;60 ml\/min\/1.73m<sup>2<\/sup> and\/or UACR 30-299 mg\/g (without comorbid DM).\u00a0Review <a href=\"https:\/\/www.med.unc.edu\/medicine\/nephrology-hypertension\/wp-content\/uploads\/sites\/1307\/2024\/11\/CKD20Management-combined.pdf\">CKD management goals<\/a> and<a href=\"https:\/\/www.med.unc.edu\/medicine\/nephrology-hypertension\/wp-content\/uploads\/sites\/1307\/2024\/11\/CKD-Care-Pathway_workup.pdf\"> CKD carepathways.\u00a0<\/a><\/li>\n<li>Minor electrolyte abnormalities. Review <a href=\"https:\/\/www.med.unc.edu\/medicine\/nephrology-hypertension\/wp-content\/uploads\/sites\/1307\/2024\/11\/Hyperkalemia.pdf\">hyperkalemia management.\u00a0<\/a><\/li>\n<li>Use of SGLT2i in CKD. <a href=\"https:\/\/www.med.unc.edu\/medicine\/nephrology-hypertension\/wp-content\/uploads\/sites\/1307\/2024\/11\/SGLT2-Inhibitor-Guideline.pdf\">Review guidelines.<\/a><\/li>\n<li>Risk of use of Iodine or Gadolinium-based contrast in CKD<\/li>\n<li>Medications use in CKD<\/li>\n<li>Perioperative medications management with CKD<\/li>\n<li>Hyperuricemia\/Gout and CKD<\/li>\n<li>Use of Cystatin C vs. Creatinine vs. measured GFR estimation<\/li>\n<li>Hypertension \u2013 uncontrolled on &lt; 3 medications<\/li>\n<li>CKD stage II in pregnancy (eGFR &gt; 60ml\/min prior to pregnancy or Cr &lt; 1.0mg\/dl before 20 weeks)<\/li>\n<li>Proteinuria less than 0.5g by 24hr urine or urine protein-to-creatinine ratio in pregnancy<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>E-consults are for focused provider-to-provider communications regarding clinical questions that a nephrologist can answer using solely the information available in EPIC, without seeing the patient.
