This helps ensure that all patients who see us will benefit from the visit.<\/p>\n<h2 style=\"font-weight: 400\">Before placing a referral<\/h2>\n<p style=\"font-weight: 400\">Please review our <a href=\"https:\/\/www.med.unc.edu\/medicine\/pulmonary\/patient-care\/pulmonary-subspecialty-care\/\">Pulmonary Subspecialty Care<\/a> page which outlines the conditions we are able to best care for.<\/p>\n<h2>Information to include with referral<\/h2>\n<p style=\"font-weight: 400\">To make the visit most informative, we ask that you please include:<\/p>\n<ul>\n<li style=\"font-weight: 400\">Specific referral diagnoses or symptoms as this determines what PFTs will be ordered<\/li>\n<li style=\"font-weight: 400\">Reports of chest imaging from the last 5 years (along with images via PowerShare or on CD \u2013 link to radiology for patients page)<\/li>\n<li>Results of all available pulmonary function tests<\/li>\n<li>Any other relevant laboratory tests<\/li>\n<\/ul>\n<h2>How To Place A Referral<\/h2>\n<h3>Use EPIC to refer a patient<\/h3>\n<p>If you are a <strong>provider within UNC Health<\/strong>, please place the referral through EPIC. Choose \u201cAmbulatory Referral to Pulmonology\u201d and select \u201cUNC Pulmonology at Eastowne\u201d.<\/p>\n<h3>Use EPIC CareLink to refer a patient<\/h3>\n<p>If you are a <strong>non-UNC physician or mid-level provider<\/strong>, you can use the UNC CareLink portal to directly place referrals, and access information about your patients (including scheduled appointments, clinical notes, and more).<\/p>\n<p>Find more information on \u202f<a href=\"https:\/\/www.med.unc.edu\/medicine\/gi\/wp-content\/uploads\/sites\/1267\/2021\/12\/PDF-MedCtr-UNC-CareLink-Information.pdf\">UNC Health<\/a>\u00a0and\u202f<a href=\"http:\/\/www.unccarelink.org\/\"><span data-contrast=\"none\">enroll for UNC CareLink here<\/span><\/a>.<\/p>\n<h3>Use a faxed referral to refer a patient<\/h3>\n<p><strong>If you are a non-UNC physician or mid-level provider and are not using the UNC CareLink portal<\/strong>, please fax a <a href=\"https:\/\/www.med.unc.edu\/medicine\/pulmonary\/wp-content\/uploads\/sites\/1321\/2024\/10\/UNC-Health-Pulmonary-Referral-Form-2024-FIllable_Oct2024.pdf\">completed referral form<\/a> or send with the patient all pertinent records, including reports of chest imaging (along with images via powershare or on CD), pulmonary function tests, and any other relevant laboratory tests.<\/p>\n<div>\n<p style=\"text-align: right\"><a class=\"btn btn-primary btn-lg  oscitas-bootstrap-container\" target=\"_self\" href=\" https:\/\/www.med.unc.edu\/medicine\/pulmonary\/wp-content\/uploads\/sites\/1321\/2024\/10\/UNC-Health-Pulmonary-Referral-Form-2024-FIllable_Oct2024.pdf\">Referral Form<\/a>\n<\/div>\n<h3>E-Consults<\/h3>\n<p><strong>For physicians within the UNC System<\/strong>, if you have a focused question that specialists can answer using solely the information available in Epic without seeing the patient, consider placing an e-consult.
