DESCRIPTION:Dr. Bharathi Zvara is an assistant professor in the Department of Maternal and Child Health at the Gillings School of Global Public Health.  Her research examines the enduring impact of early experiences on social and emotional development across the lifespan. Guided by family systems and developmental psychopathology frameworks\, her work investigates the processes through which factors within the family system (e.g.\, mother-child relationships\, childhood trauma\, and maternal depression) influence the development of self-regulation\, emotions\, and behaviors in offspring. Her current applied research examines (1) how adversity broadly\, and childhood trauma in particular\, influences developmental pathways toward psychopathology and competence across multiple generations\, (2) how the quality of interactions between parents and young children impacts risk for poor self-regulation and childhood obesity\, particularly for children born preterm\, and\,  (3) how web-based programs may accelerate science to practice for adults with childhood trauma histories.

DESCRIPTION:Patient-generated health data—such as data from wearables and patient-reported outcomes and other emerging technologies—have much potential for improving efficiency\, quality\, and outcomes. Dr. Chung will review the state of the art in PGHD\, discuss challenges in implementation into clinical care\, highlight informatics methods development for PGHD from her NIH-funded research\, and discuss opportunities for PGHD in remote monitoring and research at UNC. \nArlene Chung is a clinical informatics physician scientist (internal medicine and pediatrics) and board certified in clinical informatics. She serves as the Associate Director of Health and Clinical Informatics at the UNC School of Medicine and as the Lead Informatics Physician for patient engagement. She also is the founding fellowship director for UNC’s Clinical Informatics Fellowship Program.

SUMMARY:Medicine Grand Rounds\, Herman Pontzer "Healthy As a Hunter-Gatherer: Sorting Paleofact from Paleofiction in Modern Metabolic Medicine"
DESCRIPTION:Hunter-gatherer populations are remarkably healthy\, with men and women living into their 60’s\, 70’s and beyond without hypertension\, diabetes\, or other cardiometabolic disease. Dr. Pontzer will review his recent work on the diet\, activity\, and health of these remarkable populations. These societies are incredibly active\, but burn fewer calories and eat more carbohydrate-rich diets than generally thought. He will discuss the implications of this work for shaping public health strategies to combat cardiometabolic disease. \nDr. Pontzer investigates the physiology of humans and apes to understand how ecology\, lifestyle\, diet\, and evolutionary history affect metabolism and health. He also is interested in how ecology and evolution influence musculoskeletal design and physical activity. Field projects focus on small-scale societies\, including hunter-gatherers and subsistence farmers\, in Africa and South America. His lab researches energetics and metabolism\, including respirometry and doubly labeled water methods. \nHerman Pontzer is Associate Professor of Evolutionary Anthropology at Duke University in the Trinity College of Arts and Sciences. HIs writing appears in Scientific American\, The New York Times\, and on Twitter.

DESCRIPTION:In the department’s annual Chief Residents’ Lecture\, Hannah Coletti\, Grace Prince\, Alison Raybould\, Benjamin Robey and Benjamin Sines will present innovative projects and ideas: \nGrace Prince Evaluation of a Humanities Curriculum to Promote Higher Level Clinical Reasoning at Internal Medicine Morning Report: Results of a Qualitative Analysis  Within medical education\, the humanities are frequently used to teach communication\, promote physician wellness\, and help physicians grasp the interpersonal context of their work. Dr. Prince will describe the implementation of an innovative curriculum\, using the humanities as a tool to catalyze higher level clinical reasoning. \nHannah Coletti and Benjamin Robey A Professional Development Curriculum for Residents  Internal Medicine and combined Med-Peds residents have transitioned successfully to fellowship and employment opportunities following graduation. However\, our trainees have cited more robust support for career development and mentorship as an area for improvement. A Professional Development Curriculum was  designed and launched this year to more proactively address these needs. Drs. Coletti and Robey will present baseline resident insights on existing gaps\, and describe strategies for implementing change. \nAlison Raybould and Benjamin Sines Optimal Team Structure on Residency Teaching Services  While duty hours have been a focus of medical educators and the ACGME in recent years\, the ideal allocation of time for education\, provision of care\, and patient safety has yet to be established. Academic centers with training programs similar to UNC’s have trialed different ward team structures to identify the optimal learning environment. With the creation of UNC Hillsborough Hospital and the introduction of a different team structure there\, this question was investigated within the context of our own Medicine residency. Drs. Raybould and Sines will present their findings and identify areas for potential reform. \n

SUMMARY:Medicine Grand Rounds\, Sarah McGill "C. Diff. Now: Diagnosing\, Treating\, and Repopulating the Gut with Fecal Transplant"
DESCRIPTION:Sarah McGill\, MD\, Msc\, specializes in helping patients with recurrent C. difficile infections.\nC. difficile Infection is the most common cause of infectious\, antibiotic-associated diarrheal illness in the United States and a major cause of morbidity and mortality\, particularly in cases of recurrent or severe infection.  The standard of care in management of C. difficile infection has shifted dramatically in the last year.  I will discuss current diagnosis and management\, including how C. difficile is diagnosed at UNC\, antibiotic and novel medical therapies\, and fecal microbiota transplantation.   I will review how fecal microbiota transplantation is performed\, the proposed mechanisms behind its efficacy\, and future potential uses beyond C. difficile. \nSarah McGill cares for patients with a broad spectrum of gastrointestinal conditions\, including colon polyps\, gastrointestinal bleeding\, gastrointestinal infections\, heartburn\, abdominal pain\, dyspepsia\, inflammatory bowel disease\, constipation and diarrhea\, among others. She has a particular expertise in advanced techniques for colon polyp detection\, differentiation and resection during colonoscopy. She has expertise in treating recurrent C. difficile colitis and performs fecal transplant.

SUMMARY:Medicine Grand Rounds: Felicia Cosman\, Professor of Clinical Medicine\, Columbia University\, "Optimal Treatment Strategies for Patients at High Risk of Fracture"
DESCRIPTION:Felicia Cosman is Professor of Clinical Medicine at Columbia University College of Physicians and Surgeons and Medical Director of the National Osteoporosis Foundation. \nDr. Cosman’s research includes studies on the causes of osteoporosis and how estrogen\, SERMs\, vitamin D\, bisphosphonates\, denosumab\, and anabolic therapies work as treatments for osteoporosis. She has also studied the causes of stress fractures and factors influencing the acquisition of peak bone mass during youth\, as well as identifying patient factors\, which should prompt targeted screening for vertebral fractures. A major research focus over the last decade has been on the use of bone-building medications (teriparatide\, abaloparatide and romosozumab).  She is a major thought leader in studies of treatment sequences and combinations\, long-term treatment paradigms and goal-directed osteoporosis therapy. \nDr. Cosman is the recipient of multiple research grants from the NIH\, the Department of Defense\, the National Multiple Sclerosis Society\, and multiple pharmaceutical companies. She has published more than 150 peer-reviewed articles in the field of osteoporosis. Most recently\, she was selected as Editor-in-Chief of the journal Osteoporosis International.\nWatch the video
