This means patients grappling with Inflammatory Bowel Disease (IBD) must return multiple times to collect the necessary data for managing their treatment regimen. However, a new wave of technological advancements and streamlined training processes are providing GI specialists the ability to assess treatment efficacy and disease progression within minutes.<\/p>\n<figure id=\"attachment_66807\" class=\"thumbnail wp-caption alignright\" style=\"width: 308px\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-66807\" src=\"https:\/\/www.med.unc.edu\/medicine\/wp-content\/uploads\/sites\/945\/2024\/01\/long-using-US.jpg\" alt=\"\" width=\"298\" height=\"397\" \/><figcaption class=\"caption wp-caption-text\">Dr. Long using ultrasound to explore the bowel.<\/figcaption><\/figure>\n<p style=\"font-weight: 400\"><div class=\"row  oscitas-bootstrap-container\">\n<div class=\"col-lg-7 col-md-12 col-xs-12 col-sm-12 oscitas-bootstrap-container\">\nGastroenterologists, including the University of North Carolina\u2019s <a href=\"https:\/\/www.med.unc.edu\/medicine\/gi\/people\/millie-d-long-md-mph\/\">Millie Long, MD, MPH<\/a>, and <a href=\"https:\/\/www.med.unc.edu\/medicine\/gi\/people\/edward-l-barnes-md-mph\/\">Edward Barnes, MD, MPH<\/a>, are undergoing rigorous training to introduce intestinal ultrasound (IUS) into their repertoire of imaging techniques. UNC is expected to be the first site in the Southeast to offer this cutting-edge management tool.<\/div>\n<div class=\"col-lg-5 col-md-12 col-xs-12 col-sm-12 oscitas-bootstrap-container\">\n<\/div>\n<\/div>\n<h3 style=\"font-weight: 400\">Introducing Intestinal Ultrasound<\/h3>\n<p style=\"font-weight: 400\">Before, physicians managing IBD relied on stool and blood markers, pediatric growth indicators, and endoscopic signs of bowel healing. Starting from the lumen (middle of the intestine), gastroenterologists can meticulously measure each layer, including the submucosa, muscularis propria, deep mucosa, and more, providing a comprehensive evaluation of inflammation and how the bowel is responding to treatment.<\/p>\n<figure id=\"attachment_66809\" class=\"thumbnail wp-caption aligncenter\" style=\"width: 696px\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-66809\" src=\"https:\/\/www.med.unc.edu\/medicine\/wp-content\/uploads\/sites\/945\/2024\/01\/video-loop-US.png\" alt=\"\" width=\"686\" height=\"419\" srcset=\"https:\/\/www.med.unc.edu\/medicine\/wp-content\/uploads\/sites\/945\/2024\/01\/video-loop-US.png 822w, https:\/\/www.med.unc.edu\/medicine\/wp-content\/uploads\/sites\/945\/2024\/01\/video-loop-US-300x183.png 300w, https:\/\/www.med.unc.edu\/medicine\/wp-content\/uploads\/sites\/945\/2024\/01\/video-loop-US-768x469.png 768w, https:\/\/www.med.unc.edu\/medicine\/wp-content\/uploads\/sites\/945\/2024\/01\/video-loop-US-600x366.png 600w, https:\/\/www.med.unc.edu\/medicine\/wp-content\/uploads\/sites\/945\/2024\/01\/video-loop-US-301x184.png 301w\" sizes=\"auto, (max-width: 686px) 100vw, 686px\" \/><figcaption class=\"caption wp-caption-text\">Using a Doppler, blood flow can be identified by red and blue coloration.<\/figcaption><\/figure>\n<p style=\"font-weight: 400\">In IBD patients with bowel thickness, \u00a0a specialized Doppler enhances diagnostics. Gastroenterologists can identify blood flow, visualized as blue and red signals on ultrasound, effectively differentiating active inflammation causing the bowel to appear thick from scarring.<\/p>\n<p style=\"font-weight: 400\">Moreover, this advanced imaging technique enables the detection of abscesses, inflammatory fat, and loss of preservation of the bowel wall layer stratification. The IUS can be completed in less than 10 minutes during a routine care visit. Ultrasound is also beginning to emerge as an endpoint in IBD clinical trials. With these insights, gastroenterologists can refine treatment plans and tailor interventions to address specific manifestations of IBD.<\/p>\n<p style=\"font-weight: 400\">The new care model does require clinics to purchase an expensive ultrasounds machine with high quality B-Mode, Doppler effect and two transducers. Training gastroenterologists to use the technology can also be a large financial investment. A generous donation from The Loewenthal Family Fund has supported the implementation of IUS at UNC.<\/p>\n<p>\u201cThe power of philanthropy in medicine is evident by the development of this innovative intestinal ultrasound program,&#8221; Dr. Long said. &#8220;Gifts such as the one from the Lowenthal Family Fund will change lives for the better, and allow UNC to provide cutting edge care to our population of patients living with IBD.\u201d<\/p>\n<h3 style=\"font-weight: 400\">Insight from Europe<\/h3>\n<p style=\"font-weight: 400\">Although this imaging technique is not common practice in America, specialists in Europe have been using ultrasound as an imaging modality for patients with IBD for a decade. Through overseas studies patients have expressed the use of IUS helps them:<\/p>\n<ul>\n<li>Learn more about the disease<\/li>\n<li>Understand where symptoms come from<\/li>\n<li>Make informed decisions about managing their IBD<\/li>\n<\/ul>\n<p style=\"font-weight: 400\">\u201cSome of the medications can have complications or side effects,\u201d Dr. Long, Professor of Medicine said. \u201cWhen you\u2019re showing a patient their inflamed bowel wall on ultrasound while explaining why we need to modify treatment, it helps them make an informed decision.\u201d<\/p>\n<p style=\"font-weight: 400\">The International Bowel Ultrasound Group, based in Europe, has created a certification program for gastroenterologists interested in learning IUS techniques. Comprised of three modules, the training requires three days of workshopping followed by up to four weeks of hands-on training at a certified center. Finally, specialists must demonstrate the ability to use IUS at an advanced workshop and through testing.<\/p>\n<p style=\"font-weight: 400\">The University of Chicago, Mount Sinai Health System and Houston Methodist Hospital are currently the three certified sites actively using this technology in the United States. Drs. Long and Barnes are set to complete their training in May. <a href=\"https:\/\/www.med.unc.edu\/medicine\/gi\/people\/hans-herfarth-md-phd\/\">Hans Herfarth, MD, PhD,<\/a> Director of the IBD Center at UNC, is expected to be certified in 2025.<\/p>\n<h3 style=\"font-weight: 400\">Putting the Probe in Program<\/h3>\n<p style=\"font-weight: 400\"><a href=\"https:\/\/www.med.unc.edu\/medicine\/gi\/patient-care\/clinical-services\/unc-center-for-inflammatory-bowel-disease\/\">UNC\u2019s IBD center<\/a> stands as one of the largest in the country, with over 3,000 patient visits from across the Southeast every year. Limiting the number of times someone has to return for testing would pave the way to more efficient and personalized care.<\/p>\n<p style=\"font-weight: 400\">The non-invasive nature and lack of radiation makes IUS especially valuable for pregnant and pediatric patients.<\/p>\n<p style=\"font-weight: 400\">Dr. Long envisions IUS training being implemented into the UNC Division of Gastroenterology\u2019s Fellowship Program.
