In addition, you will need to fill out a form so that your key card can be programmed to open both the building and the laboratory locks. Please <a href=\"https:\/\/www.med.unc.edu\/microscopy\/wp-content\/uploads\/sites\/1393\/2023\/11\/nights-and-weekends-empty-form-with-checks.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">download the form<\/a> and complete all mandatory items in the top section. Do not sign at the bottom of the form. Once complete, please <a href=\"mailto:msl@med.unc.edu\">email the completed form to the MSL staff email<\/a>. Do not bring us a printed form; this slows down the process.
