How to Refer a Patient:<\/h2>\n

Providers who have access to UNC CareLink can submit a referral online.<\/p>\n

If you would like to enroll, click on the link and select “Request an Account”. You may wish to download an overview<\/a> of the referral portal or view a step-by-step guide<\/a> on how to sign up<\/span>.<\/p>\n

If you do not have a UNC CareLink account, please fax a completed referral form to the appropriate fax number (see below).<\/p>\n
