{"id":8358,"date":"2020-04-23T10:47:32","date_gmt":"2020-04-23T14:47:32","guid":{"rendered":"https:\/\/www.med.unc.edu\/neurology\/?page_id=8358"},"modified":"2023-12-15T13:16:06","modified_gmt":"2023-12-15T18:16:06","slug":"traumatic-brain-injury-research-boost3","status":"publish","type":"page","link":"https:\/\/www.med.unc.edu\/neurology\/divisions\/neurocritical-care\/neurocritical-care-research\/traumatic-brain-injury-research-boost3\/","title":{"rendered":"Traumatic Brain Injury Research: BOOST3"},"content":{"rendered":"<h2>What is TBI?<\/h2>\n<p><strong>T<\/strong>raumatic <strong>B<\/strong>rain <strong>I<\/strong>njury <strong>(TBI)<\/strong> is sudden damage to the brain caused by an outside force to the head \u2013 such as a car crash, a fall, or something hitting the head.<\/p>\n<ul>\n<li>Every 15 seconds someone in the US suffers a major TBI.<\/li>\n<li>Every five minutes someone is forever disabled as a result of TBI.<\/li>\n<li>TBI is the leading cause of death and disability in children and adults 1-44 years of age.<\/li>\n<\/ul>\n<p>TBI can affect a person\u2019s ability to think and remember things, cause problems with balance and coordination, prevent a person from functioning independently, and cause permanent brain damage or even death.<\/p>\n<h2><strong><a href=\"https:\/\/www.med.unc.edu\/neurology\/wp-content\/uploads\/sites\/716\/2020\/04\/boost-3_logo.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignright wp-image-8353\" src=\"https:\/\/www.med.unc.edu\/neurology\/wp-content\/uploads\/sites\/716\/2020\/04\/boost-3_logo.jpg\" alt=\"BOOST3 Logo\" width=\"250\" height=\"38\" srcset=\"https:\/\/www.med.unc.edu\/neurology\/wp-content\/uploads\/sites\/716\/2020\/04\/boost-3_logo.jpg 1200w, https:\/\/www.med.unc.edu\/neurology\/wp-content\/uploads\/sites\/716\/2020\/04\/boost-3_logo-300x46.jpg 300w, https:\/\/www.med.unc.edu\/neurology\/wp-content\/uploads\/sites\/716\/2020\/04\/boost-3_logo-1024x155.jpg 1024w, https:\/\/www.med.unc.edu\/neurology\/wp-content\/uploads\/sites\/716\/2020\/04\/boost-3_logo-768x116.jpg 768w, https:\/\/www.med.unc.edu\/neurology\/wp-content\/uploads\/sites\/716\/2020\/04\/boost-3_logo-600x91.jpg 600w\" sizes=\"auto, (max-width: 250px) 100vw, 250px\" \/><\/a><\/strong>What is BOOST3?<\/h2>\n<p>BOOST3 is a research study to learn if either of two strategies for monitoring and treating patients with TBI in the intensive care unit (ICU) is more likely to help them get better.\u00a0 Both of these alternative strategies are used in standard care.\u00a0 It is unknown if one is more effective than the other.\u00a0 In one strategy doctors concentrate only on preventing high ICP (intracranial pressure) caused by a swollen brain.\u00a0 In the other strategy doctors try to prevent high ICP, and also try to prevent low PbtO2 (brain oxygen).\u00a0 It is unknown if measuring and treating low brain oxygen is more effective, less effective, or the same as monitoring and treating high brain pressure alone.\u00a0 The results of this study will help doctors discover if one of these methods is more safe and effective.<\/p>\n<h2>Who will be included?<\/h2>\n<ul>\n<li>People who are 14 years or older <u>with a<\/u><\/li>\n<li>Blunt closed head injury, <u>with<\/u><\/li>\n<li>Severe brain injury, <u>and<\/u><\/li>\n<li>Can start the study immediately following brain monitor placement.<\/li>\n<\/ul>\n<p>People who meet the entry criteria will be randomly entered, like flipping a coin, into one of the two study groups:<\/p>\n<ul>\n<li>Those that get medical care based on monitoring of pressure in the brain (intracranial pressure or ICP) alone.<\/li>\n<li>Those who get medical care based on both ICP and the amount of oxygen in the brain (brain tissue oxygen or PbtO2).<\/li>\n<\/ul>\n<h2>What are the benefits?<\/h2>\n<p>Because we do not know which treatment is best for treating TBI, a person enrolled in the study may benefit from being placed in one study group over the other.\u00a0 Based on the information we get from this study, people who have a TBI in the future may benefit from what is learned from this study.<\/p>\n<h2>What are the risks?<\/h2>\n<p>The different treatment strategies may affect:<\/p>\n<ul>\n<li>Risk of pneumonia or lung injury<\/li>\n<li>Severe infection in the blood or brain<\/li>\n<\/ul>\n<p>Brain probes may involve risks of<\/p>\n<ul>\n<li>Bleeding or infection<\/li>\n<\/ul>\n<p>Risks of participating in research include:<\/p>\n<ul>\n<li>Breaches of confidentiality<\/li>\n<\/ul>\n<h2>How is enrollment in BOOST3 different from other studies?<\/h2>\n<p>Normally, researchers get permission (consent) before a person can be included in a study.\u00a0 A person with a severe TBI will not be able to give consent at the time of injury. Since TBI must be treated quickly, there might not be enough time to locate and talk to the person\u2019s family or legal representative about the study.\u00a0 The strategies being studied typically need to start within 2 to 10 hours of injury.\u00a0 When consent is not possible, a person might be enrolled in this study without consent.\u00a0 This is called \u201c<strong>E<\/strong>xception<strong> f<\/strong>rom <strong>I<\/strong>nformed <strong>C<\/strong>onsent\u201d (<strong>EFIC)<\/strong>.\u00a0 Once the family or legal representative is located, they will be asked whether they want the participant to continue in the study.<\/p>\n<h2>What is EFIC?<\/h2>\n<p>Exception from informed consent (EFIC) for emergency research refers to a special set of rules used by the US government to regulate studies when research participants cannot tell researchers their desires in a medical emergency.\u00a0 These special rules allow research studies in certain emergency situations to be conducted without consent.<\/p>\n<p>EFIC can <u>only<\/u> be used when:<\/p>\n<ul>\n<li>The person\u2019s life is at risk, AND,<\/li>\n<li>The best treatment is not known, AND<\/li>\n<li>The study might help the person, AND<\/li>\n<li>It is not possible to get permission:\n<ul>\n<li>from the person because of his or her medical condition nor<\/li>\n<li>from the person\u2019s representative because there is a very short amount of time required to treat the medical problem, or the representative is not available.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h2>How can I share my opinions about this study?<\/h2>\n<p>Before the study starts, meetings will be held in the community to provide information, answer questions, and get community members\u2019 thoughts and feelings about the study.\u00a0 You can call the study team to complete a one-on-one interview or complete an <a href=\"http:\/\/bit.ly\/BOOST3CommunitySurvey\">online survey<\/a> about the study.\u00a0 There will also be information about the study in the media (for example, newspapers, TV and radio).<\/p>\n<h2>What if I do not want to be included in the study?<\/h2>\n<p>If you decide you don\u2019t want to be included in the event and you suffer a future TBI, contact us to request an Opt Out medical alert bracelet be sent to you to wear with the words \u201cBOOST3 declined.\u201d\u00a0 Wearing this medical alert bracelet at all times throughout the study period (about 5 years) is your way of communicating your wishes in case you suffer a severe TBI and are unconscious.
