Diplomas

The UNC School of Medicine Registrar’s Office provides certified copies of diplomas that can be requested by completing the SOM Consent Form (electronic Form or PDF Version for Mailing) and returning it to somrecords@med.unc.edu. There is no fee to request a certified copy of diploma.

To request a diploma granted prior to 2007 and/or to request a replacement diploma, please contact the main campus Registrar’s Office.
