In order to achieve the goal of the Care Coordination Program, applicants are required to coordinate the health care delivery services in rural communities through the following four focus areas.<\/p>\n<p>&nbsp;<\/p>\n<p>Literature and previous award recipients have cited these key elements for a successful rural care coordination program:<\/p>\n<ol>\n<li><strong><u>Collaboration:<\/u><\/strong> Utilizing a collaborative approach to coordinate and deliver health care services through a consortium, in which member organizations actively engage in integrated, coordinated, patient-centered delivery of health care services.<\/li>\n<li><strong><u>Leadership and Workforce:<\/u><\/strong> Developing and strengthening a highly skilled care coordination workforce to respond to vulnerable populations\u2019 unmet needs within the rural communities.<\/li>\n<li><strong><u>Improved Outcomes:<\/u><\/strong> Expanding access and improving care quality and delivery, and health outcomes through evidence-based model and\/or promising practices tailored to meet the local populations\u2019 needs.<\/li>\n<li><strong><u>Sustainability:<\/u><\/strong> Developing and strengthening care coordination program\u2019s financial sustainability by establishing effective revenue sources such as expanded service reimbursement, resource sharing, and\/or contributions from partners at the community, county, regional, and state levels.<\/li>\n<\/ol>\n<ul>\n<li><strong>Successful award recipients of the Care Coordination Program will be able to demonstrate: <\/strong>\n<ul>\n<li>Enhanced integrated systems to collaborate and share data among member organizations<\/li>\n<li>Effective care coordination workforce to meet needs within the rural communities<\/li>\n<li>Improved access, delivery, and quality of services and overall patients\u2019 health outcomes<\/li>\n<li>Increased program financial sustainability.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Eligibility<\/p>\n<p>&nbsp;<\/p>\n<p>Applicants must be:<\/p>\n<ul>\n<li>Located in a rural county or eligible rural census tract<\/li>\n<li>Rural public and nonprofit private entities including faith-based and community organizations<\/li>\n<li>In a consortium with at least two additional organizations. These two other organizations can be rural, urban, nonprofit or for-profit. The consortium must include at least three or more health care providers.<\/li>\n<li>Have not previously received an award for the same or similar project unless the applicant is proposing to expand the scope of the project or the area that will be served through the project.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>HRSA Review\u202fCriteria<\/p>\n<p>&nbsp;<\/p>\n<p>These criteria are the basis upon which the reviewers will evaluate and score the merit of the application.
