UNC faculty and care team members will present at the Fifth Annual Stroke Survivor & Caregiver Event, hosted by the UNC Comprehensive Stroke Program.

This free event brings together stroke survivors, caregivers, and healthcare professionals for a meaningful day of fellowship, inspiration, and education. Attendees will hear from expert speakers, participate in discussions, and connect with others in the community.

Dr. John Baratta, Medical Director of Stroke Rehabilitation, and Kate Cardoza, MSW, LCSW, will present on CREST: Coordinating Resources to Enhance Stroke Transitions funded by the Paul Coverdell National Acute Stroke Program during a research session.

CREST aims to develop a sustainable model for identifying unmet social needs among stroke survivors and connecting them with timely, appropriate resources. Researchers are testing improved workflows, referral pathways, and data-tracking processes across multiple clinical sites. The goal is to demonstrate that coordinated discharge transitions lead to better functional outcomes, fewer gaps in care, and improved experiences for patients and caregivers.

In addition, Dr. Baratta will lead a demonstration focused on adaptive equipment in stroke recovery.
