Attendees will hear from expert speakers, participate in discussions, and connect with others in the community.<\/p>\n<p data-start=\"585\" data-end=\"811\">Dr. John Baratta, Medical Director of Stroke Rehabilitation, and Kate Cardoza, MSW, LCSW, will present on <strong data-start=\"691\" data-end=\"754\">CREST: Coordinating Resources to Enhance Stroke Transitions<\/strong> funded by the Paul Coverdell National Acute Stroke Program during a research session.<\/p>\n<p data-start=\"813\" data-end=\"1267\">CREST aims to develop a sustainable model for identifying unmet social needs among stroke survivors and connecting them with timely, appropriate resources. Researchers are testing improved workflows, referral pathways, and data-tracking processes across multiple clinical sites.
