Please refer to the current SOM Photo ID Badge Request Procedure for all requirements and delivery options (see link on left).<\/p>\n<p>For a new badge or changes to a current badge, use the <a href=\"https:\/\/www.med.unc.edu\/planning\/id-badges-building-access\/photo-id-badge-request-form\/\">Photo ID Badge Form.<\/a><\/p>\n<ul>\n<li>Fill out online badge request form completely.<\/li>\n<li>Form must have authorized signature from your Department.<\/li>\n<li>Have the Department head or designee email the signed form + passport quality color photo (solid neutral background, no outdoor photos or scenery) to the Photo ID Badge Office.<\/li>\n<li>Photos must be in JPG, PNG, or PDF format.<\/li>\n<\/ul>\n<p><strong><span style=\"color: #ab1b1b\">PLEASE FILL OUT ALL FORMS COMPLETELY TO AVOID PROCESSING DELAYS.\u00a0<\/span><\/strong><\/p>\n<p><b>Restricted Access<\/b>: Access to Comparative Medicine,\u00a0Irradiator Rooms, Select Agent Facilities, and other restricted areas may require additional authorizations.
