{"id":15251,"date":"2022-12-01T10:39:32","date_gmt":"2022-12-01T15:39:32","guid":{"rendered":"https:\/\/www.med.unc.edu\/psych\/cecmh\/?page_id=15251"},"modified":"2024-11-25T12:16:05","modified_gmt":"2024-11-25T17:16:05","slug":"familiar-faces","status":"publish","type":"page","link":"https:\/\/www.med.unc.edu\/psych\/cecmh\/about-us\/development\/familiar-faces\/","title":{"rendered":"Familiar Faces"},"content":{"rendered":"<h2><strong><span id=\"page144R_mcid0\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Wake County Familiar Faces Health Collaborative<\/span><\/span><\/strong><\/h2>\n<p>A collaboration project by the <a href=\"https:\/\/clinicalscholarsnli.org\/projects\/wake-county-familiar-faces-health-collaborative\/\" target=\"_blank\" rel=\"noopener\">Robert Wood Johnson Foundation <span id=\"page144R_mcid23\"><span dir=\"ltr\" role=\"presentation\">Clinical Scholar<\/span><\/span> Program<\/a> designed <span id=\"page144R_mcid23\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">to engaged and coordinate with community stakeholders to provide <\/span><span dir=\"ltr\" role=\"presentation\">integrated health services to vulnerable individuals with complex needs and who <\/span><span dir=\"ltr\" role=\"presentation\">have frequent interaction with crisis systems because current systems are unable to <\/span><span dir=\"ltr\" role=\"presentation\">meet their needs. <\/span><\/span><\/p>\n<p><span id=\"page144R_mcid23\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Recent literature shows that the lack of integrated services for <\/span><span dir=\"ltr\" role=\"presentation\">those most vulnerable within a community leads to worse outcomes and costly <\/span><span dir=\"ltr\" role=\"presentation\">human suffering. The RWJ Clinical Scholars team members all work for organizations <\/span><span dir=\"ltr\" role=\"presentation\">(Wake County Government, WakeMed Health &amp; Hospitals, Duke University Health <\/span><span dir=\"ltr\" role=\"presentation\">System, and UNC Health) with deep roots in the community. Individually and <\/span><span dir=\"ltr\" role=\"presentation\">collectively, they work with and among networks of community stakeholders and <\/span><span dir=\"ltr\" role=\"presentation\">service providers who have a substantive understanding of institutional strengths and <\/span><span dir=\"ltr\" role=\"presentation\">gaps related to services for the target population. <\/span><\/span><\/p>\n<p><span id=\"page144R_mcid23\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">The team seeks to engage <\/span><span dir=\"ltr\" role=\"presentation\">vulnerable community members and front-line service providers to understand <\/span><span dir=\"ltr\" role=\"presentation\">barriers and challenges associated with service delivery and support systems for this <\/span><span dir=\"ltr\" role=\"presentation\">population. Utilizing existing data sources within our community, our team seeks to <\/span><span dir=\"ltr\" role=\"presentation\">understand what it takes to partner, integrate, and deliver a holistic care model to <\/span><span dir=\"ltr\" role=\"presentation\">familiar faces, vulnerable individuals who have frequent interactions with crisis safety-<\/span><span dir=\"ltr\" role=\"presentation\">net systems. This information will provide valuable insight to improve upon existing <\/span><\/span><span id=\"page144R_mcid24\" class=\"markedContent\"><\/span><span id=\"page1R_mcid0\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">strategies to develop an empathetic human-centered design that addresses systemic <\/span><span dir=\"ltr\" role=\"presentation\">problems to improve services and health outcomes. Their efforts will include utilize <\/span><span dir=\"ltr\" role=\"presentation\">continuous quality improvement practices to allow for ongoing improvement efforts <\/span><span dir=\"ltr\" role=\"presentation\">and broad community adoption.<\/span><\/span><\/p>\n<a class=\"btn btn-info btn-lg  oscitas-bootstrap-container\" target=\"_blank\" href=\"https:\/\/www.med.unc.edu\/psych\/cecmh\/wp-content\/uploads\/sites\/880\/2023\/11\/Toolkit-for-Familiar-Faces-Providing-Integrated-Medical-and-Social-Services-to-a-Vulnerable-Population.pdf\">Integrated Medical and Social Services Toolkit <\/a>\n<hr \/>\n<h3><strong><span id=\"page1R_mcid2\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Wicked Problem Description<\/span><\/span><\/strong><span id=\"page1R_mcid3\" class=\"markedContent\"><\/span><span id=\"page1R_mcid4\" class=\"markedContent\"><\/span><\/h3>\n<p><span id=\"page1R_mcid4\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">The \u201cWicked Problem\u201d in Wake County, like many communities across the country, is <\/span><span dir=\"ltr\" role=\"presentation\">related to the lack of integrated health services and supports to vulnerable individuals <\/span><span dir=\"ltr\" role=\"presentation\">with complex needs and who have frequent interaction with a combination of <\/span><span dir=\"ltr\" role=\"presentation\">criminal justice and correctional systems, emergency medical services (EMS), <\/span><span dir=\"ltr\" role=\"presentation\">homelessness services, physical and mental health services, substance use detox and <\/span><span dir=\"ltr\" role=\"presentation\">treatment services. Several challenges within the healthcare and social services <\/span><span dir=\"ltr\" role=\"presentation\">delivery systems combine to make this a &#8220;wicked problem&#8221;. Some of these challenges <\/span><span dir=\"ltr\" role=\"presentation\">include using stand-alone information systems with confidentiality rules that limit the <\/span><span dir=\"ltr\" role=\"presentation\">fine coordination of care and services within a community. Second, insufficient, <\/span><span dir=\"ltr\" role=\"presentation\">confusing, and chaotic access to non-emergency department crisis services,<\/span><br role=\"presentation\" \/><span dir=\"ltr\" role=\"presentation\">especially for the uninsured and indigent, without coordination across the care <\/span><span dir=\"ltr\" role=\"presentation\">system. Third, the lack of integrated health and mental health medical homes that <\/span><span dir=\"ltr\" role=\"presentation\">can provide trauma-informed care where needed. Fourth, insufficient access to <\/span><span dir=\"ltr\" role=\"presentation\">resources can address social determinants of health &#8211; most notably housing with <\/span><span dir=\"ltr\" role=\"presentation\">needed supportive services, nutritious food, transportation, employment, legal <\/span><span dir=\"ltr\" role=\"presentation\">services, and personal security. Fifth, vulnerable individuals lack consistent access to <\/span><span dir=\"ltr\" role=\"presentation\">necessary pharmaceuticals\u2014lastly, insufficient re-entry planning for individuals <\/span><span dir=\"ltr\" role=\"presentation\">leaving the county jail or state prison.<\/span><\/span><\/p>\n<h3><strong><span id=\"page1R_mcid6\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Proposed Project Plan<\/span><\/span><\/strong><\/h3>\n<p><span id=\"page1R_mcid7\" class=\"markedContent\"><\/span><span id=\"page1R_mcid8\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">The project will proceed in three (phases) throughout the grant period: <\/span><\/span><\/p>\n<p><span id=\"page1R_mcid9\" class=\"markedContent\"><\/span><span id=\"page1R_mcid10\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\"><strong>Problem definition<\/strong> \u2013 The RWJ Clinical Scholars team will work with community <\/span><span dir=\"ltr\" role=\"presentation\">stakeholders to identify the target population and better understand barriers and <\/span><span dir=\"ltr\" role=\"presentation\">challenges associated with their access to integrated, collaborative services. This <\/span><span dir=\"ltr\" role=\"presentation\">phase will include interaction with clients, their families, community members and <\/span><span dir=\"ltr\" role=\"presentation\">front-line service providers.<\/span><\/span><span id=\"page1R_mcid11\" class=\"markedContent\"><\/span><span id=\"page1R_mcid12\" class=\"markedContent\"><br role=\"presentation\" \/><\/span><\/p>\n<p><span id=\"page1R_mcid12\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\"><strong>Coherence design<\/strong> \u2013 The team will take phase one findings and use design thinking <\/span><span dir=\"ltr\" role=\"presentation\">methodologies to understand individual experiences and how services are delivered <\/span><span dir=\"ltr\" role=\"presentation\">in order to improve and develop care models that cater to this relatively small group <\/span><span dir=\"ltr\" role=\"presentation\">of individuals who suffer expensively in the shadows , align necessary service delivery <\/span><span dir=\"ltr\" role=\"presentation\">and interdependencies to improve health outcomes. Further, this phase will include <\/span><span dir=\"ltr\" role=\"presentation\">an implementation strategy including utilization of real-time feedback loops to allow <\/span><span dir=\"ltr\" role=\"presentation\">for ongoing improvement and project evaluation information.<\/span><\/span><\/p>\n<p><span id=\"page1R_mcid13\" class=\"markedContent\"><\/span><span id=\"page1R_mcid14\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\"><strong>Implementation<\/strong> \u2013 The team will work with community providers to implement <\/span><span dir=\"ltr\" role=\"presentation\">design deliverables and utilize continuous quality improvement practices to allow for <\/span><\/span><span id=\"page3R_mcid0\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">ongoing improvement efforts. Members will serve as mentors and coaches and will <\/span><span dir=\"ltr\" role=\"presentation\">help guide adjustments and improvements as the implementation progresses.<\/span><\/span><span id=\"page3R_mcid1\" class=\"markedContent\"><\/span><span id=\"page3R_mcid2\" class=\"markedContent\"><\/span><\/p>\n<h3><strong><span id=\"page3R_mcid2\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Anticipated Outcomes<\/span><\/span><\/strong><\/h3>\n<p><span id=\"page3R_mcid3\" class=\"markedContent\"><\/span><span id=\"page3R_mcid4\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Short-Term Outcomes: Define and identify the target population, available services, <\/span><span dir=\"ltr\" role=\"presentation\">barriers, and gaps in care. Work with community stakeholders to identify at-risk <\/span><span dir=\"ltr\" role=\"presentation\">individuals in our community. Design a client-centered holistic care model that <\/span><span dir=\"ltr\" role=\"presentation\">addresses the needs of this population.<\/span><\/span><\/p>\n<p><span id=\"page3R_mcid5\" class=\"markedContent\"><\/span><span id=\"page3R_mcid6\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Long-Term Outcomes: Implement an evidenced-based client-centered holistic <\/span><span dir=\"ltr\" role=\"presentation\">care model that improves our vulnerable population&#8217;s physical, mental, and social <\/span><span dir=\"ltr\" role=\"presentation\">well-being. Implement a system that identifies at-risk individuals early on to prevent <\/span><span dir=\"ltr\" role=\"presentation\">them from becoming familiar faces. Use predictive analytics to identify cost-savings <\/span><span dir=\"ltr\" role=\"presentation\">that can potentially serve other individuals at risk.<\/span><\/span><\/p>\n<h3><span id=\"page3R_mcid7\" class=\"markedContent\"><\/span><span id=\"page3R_mcid8\" class=\"markedContent\"><\/span><strong><span id=\"page3R_mcid22\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Partnerships<\/span><\/span><\/strong><\/h3>\n<p><span id=\"page3R_mcid23\" class=\"markedContent\"><\/span><span id=\"page3R_mcid24\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">The RWJ Clinical Scholars team will work a broad network of community <\/span><span dir=\"ltr\" role=\"presentation\">stakeholders and service providers who have a substantive understanding of this <\/span><span dir=\"ltr\" role=\"presentation\">population&#8217;s needs. These stakeholders include SAS, Wake County District Attorney, <\/span><span dir=\"ltr\" role=\"presentation\">Wake County Manager&#8217;s Office, Wake County Sheriff&#8217;s Office, Alliance Health, Wake <\/span><span dir=\"ltr\" role=\"presentation\">County Housing, and all three healthcare systems providing service in our region.<\/span><\/span><span id=\"page3R_mcid25\" class=\"markedContent\"><\/span><span id=\"page3R_mcid26\" class=\"markedContent\"><br role=\"presentation\" \/><\/span><\/p>\n<h3><strong><span id=\"page3R_mcid26\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Evaluation Strategies<\/span><\/span><\/strong><\/h3>\n<p><span id=\"page3R_mcid27\" class=\"markedContent\"><\/span><span id=\"page3R_mcid28\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">The information needed to support the evaluation of project outcomes will part of <\/span><span dir=\"ltr\" role=\"presentation\">the project planning process. The team will identify a complete list of variables and <\/span><span dir=\"ltr\" role=\"presentation\">metrics needed to monitor progress. These variables include the number of identified <\/span><span dir=\"ltr\" role=\"presentation\">familiar faces and their health outcomes, change in frequency of system interactions, <\/span><span dir=\"ltr\" role=\"presentation\">number of hand-offs between and among agencies across systems of care.
