REACH is an enhanced primary care clinic, meaning established patients receive services above and beyond typical primary care. Nearly 100% of clients served through the clinic have lived experience of severe mental illness, substance use disorder, incarceration, homelessness, and\/or disability. The clinic has earned the Advanced Medical Home Plus (AMH+) designation under DHHS and NC Medicaid.<\/p>\n<p>Please reach out to the REACH clinic (984-974-4832) if you have any questions or want to \/>\n<h4><strong>Enhanced services and special programs:<\/strong><\/h4>\n<ul>\n<li>Medications for Opioid Use Disorder (MOUD) treatment<\/li>\n<li>Hepatitis C Treatment Program for uninsured and underinsured patients<\/li>\n<li>Formerly Incarcerated Transition (FIT) Program, providing intensive medical care management for patients released from prison within the past 2 years<\/li>\n<li>Community Cabinet stocked with non-perishable items as well as seasonal distribution of fresh produce from UNC\u2019s Penny Lane Farm<\/li>\n<li>UNC School of Dentistry REACH Dental Clinic, offering dental screenings, cleanings, oral hygiene education, fillings, extractions, and denture adjustments<\/li>\n<\/ul>\n<hr \/>\n<h4><strong>Community Cabinet<\/strong><\/h4>\n<p>The REACH Community Cabinet supports all visitors to the primary care clinic and co-located services with basic needs distribution. Launched by a <a href=\"https:\/\/www.med.unc.edu\/psych\/centers-and-programs\/mental-health-equity-and-inclusion-program\/#:~:text=The%20DIVE%2DIN%20pilot%20grant,Cycles%201%2D3%20in%202023.\" target=\"_blank\" rel=\"noopener\">DIVE-IN grant<\/a> in January 2023, the Cabinet served over 170 individuals through more than 440 Cabinet visits and distributed over 2800 food, hygiene, and household items in its first year. This partnership supports the long-term sustainability of the Community Cabinet as well as increases the volume and range of non-perishable, fresh, and frozen food items available for distribution.
