We need your records sent to us in order to begin review of your referral. You are responsible for contacting your previous or current provider to request the records to be sent to our office. Your provider can fax your records to: <strong>984-974-9646<\/strong>\u00a0OR mail to <strong>401 East Whitaker Mill Road, Raleigh, NC 27608.<\/strong>\n<ul>\n<li><strong>If no records are received, your referral will not be reviewed and will be closed in 60 days. <\/strong><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>What happens once we receive your referral form and records?<br \/>\n<\/strong>Once we received your completed referral packet, it will be reviewed by clinic management and a determination will be made. Our referral specialist will contact you within 72 business hours to conduct pre-screening for program eligibility. SMI diagnoses include schizophrenia, schizoaffective disorder, bipolar disorder, major depression with psychotic features, psychotic disorders, etc. This process helps REACH verify each patient\u2019s SMI diagnosis but most importantly ensures the availability of psychiatric medication management and\/or supportive therapy. REACH works in partnership with psychiatric providers but does not routinely prescribe antipsychotics and other important psychiatric medications through primary care. If you are a provider and are unsure about making a referral, please call 984-974-4832.<\/p>\n<p>The <a href=\"https:\/\/www.med.unc.edu\/psych\/cecmh\/wp-content\/uploads\/sites\/880\/2024\/01\/UNC-REACH-Referral-Form.pdf\" target=\"_blank\" rel=\"noopener\">REACH Referral Form<\/a> can be faxed with <a href=\"https:\/\/www.uncmedicalcenter.org\/uncmc\/patients-visitors\/medical-records\/\" target=\"_blank\" rel=\"noopener\">records and release of information<\/a> to the intake team at 984-974-4920.
