Integrated Care team presents at national SAMHSA conference<\/strong><\/p>\n

Angela Clay, program coordinator, Princess Douglas-Williams, nurse care manager, and Robin Reed, MD, MPH, director, Integrated Care, presented \u201cPopulation health\/patient centered medical home elements\u201d on a panel at the National SAMHSA Conference in Austin, Texas in August 2017. The presentation looked at the effective implementation of services such as transitional care, chronic care management and Medicare annual wellness visits. Beat Stine, MD, MPH , assistant dean for clinical education, UNC School of Medicine; professor; director, Medical Student Programs, Department of Family Medicine, also presented \u201cReverse co-location: Case study of integrating primary care in a mental health setting.\u201d Reed says that both presentations were well received by SAMHSA leadership as well as grantees.<\/p>\n
