The patient population includes trauma survivors in the perinatal period (pregnant, up to 1 year postpartum, or post-perinatal loss). The most common types of trauma experienced by our population includes obstetrical, neonatal, sexual violence, interpersonal violence, and developmental traumas. Our treatment approach depends on patient presentation and goals. Typically, for PTSD, we stabilize (if needed) with a brief course of skills followed by Prolonged Exposure or Cognitive Processing Therapy. For acute or ongoing trauma (as with interpersonal violence, or having a child hospitalized for life threatening complications in the NICU), Acceptance and Commitment therapy, DBT skills, and Cognitive Behavioral Therapy are the interventions of choice. Subsequent work may involve an evidence-based treatment for PTSD and\/or associated comorbid disorders (e.g., panic, OCD). There is also significant opportunity to receive training on treating the intersection of trauma with grief and loss.<\/p>\n<p><em>Comprehensive Dialectical Behavior Therapy:<\/em> For individuals with complex trauma, high diagnostic complexity, and acuity, our program also offers comprehensive Dialectical Behavior Therapy (DBT). The Perinatal DBT program, directed by Dr. Tiffany Hopkins, is one of the only full-model DBT programs in the US adapted specifically for perinatal patients. The DBT program includes weekly skills group, individual therapy, phone coaching, and consultation group. Additionally, interns will have the opportunity to complete Dr. Melanie Harned\u2019s DBT+Prolonged Exposure protocol, for patients with primary PTSD diagnoses. Interns will co-facilitate the skills group with a licensed clinician, and over the course of the year-long experience, gain autonomy in planning and leading group sessions. Additionally, interns will hold at least one individual DBT case, and will provide phone coaching and participate in consultation group, depending on their level of experience and training goals.<\/p>\n<p>Interns will receive one hour of individual supervision and one hour of peer consultation each week. The intern will receive training in PE and CPT and deliver these interventions with adults. Supervision takes a developmental approach, beginning with modeling and co-facilitation, transitioning to reviewing session tapes, with the aim of clinicians achieving independent practice with supervision by the end of the training year. The intern will receive training in TF-CBT and deliver these interventions with children\/adolescents and their parents. The intern will also gain exposure to Parent-Child Interaction Therapy and behavioral therapy for managing child maladaptive behavior and enhancing parenting skills. The intern will conduct child mental health assessments including trauma assessment, developmental history, and dyadic observation, and then write up a Clinical Assessment. The intern will also provide individual therapy with children using a variety of evidence-based treatments (e.g., TF-CBT) and conduct parenting groups. Supervision is reflective and focused on case conceptualization and treatment planning. Additionally, the intern will participate in a year-long didactic series emphasizing trauma-informed care. Training and professional development goals are set at the beginning of each rotation. Professional development topics include support and coaching regarding postdoctoral and job applications. Availability of supervision training and opportunities will vary based upon 1) an intern\u2019s previous training and experience with trauma-focused assessment and intervention, and 2) whether the clinic has current graduate student clinicians. If an intern demonstrates readiness for supervision and supervisees are available, interns may participate in vertical supervision as a part of their training experience.<\/p>\n<\/div>\n\n
