But also be sure to appreciate the flexibility!\"<br\/>-<em>Class of 2020 member<\/em><\/p><\/div>\n<p>The final year of residency (PGY-4) concludes training in the General Psychiatry Residency Program and allows the resident to hone skills and pursue further educational endeavors.<\/p>\n<h2>Rotations<\/h2>\n<h3><span style=\"font-family: 'Georgia',serif;color: #505962;font-weight: normal\">Outpatient Clinic, Clinic Selectives<\/span><\/h3>\n<p style=\"background: white\"><span style=\"font-family: 'Arial',sans-serif;color: #4f4f4f\">This year includes an ongoing commitment to outpatient work with psychotherapy and psychopharmacology patients. Residents will continue to follow continuity patients for medication management and psychotherapy in the outpatient clinic. Additionally they will have outpatient psychiatry rotation opportunities with Substance Treatment and Recovery (STAR) clinic, Psychiatry Consultation Clinic (PCC), and the Taking Care of Our Own (TCOOO) Program. In STAR clinic they will manage substance abuse patients and perform new patient evaluations. In PCC, they will see patients referred by providers from family medicine and internal medicine for targeted assessments and recommendations. In TCOOO, they will have the opportunity to provide psychiatric care for other providers at UNC. <\/span><\/p>\n<h3>AHEC<\/h3>\n<p>An average of one day per week is dedicated to an elective choice of an advanced community psychiatry assignment through the\u00a0<a href=\"http:\/\/www.med.unc.edu\/psych\/education\/ahec\">North Carolina Area Health Education Center<\/a> (AHEC) program.\u00a0This may include clinical work with special populations (substance abuse, adolescents, criminal justice, rural services, mobile crisis, assertive community treatment teams, etc.) or it may focus on administrative issues within the community or research interests.<\/p>\n<h3>Collaborative Care<\/h3>\n<p>Residents have opportunities\u00a0in\u00a0both\u00a0integrated\u00a0care and collaborative care\u00a0models\u00a0in\u00a0a\u00a0variety of primary care and specialty care settings.\u00a0 In collaborative care, residents\u00a0provide\u00a0consultation\u00a0services\u00a0through weekly case review with the care manager, as well as support to providers and\u00a0make treatment recommendations when questions arise. Overall, the rotation will provide experience in consultation\u00a0and case review with a care manager, development of liaison skills through provider consultation, working within a population health framework, and opportunities to be involved with quality improvement\u00a0and\/or\u00a0educational\u00a0projects\u00a0in the\u00a0primary\u00a0care\u00a0setting.<\/p>\n<h3>Acting Attending<\/h3>\n<p>As a senior resident in the training program, the PGY-4 will also spend two blocks as an Acting Attending. During these segments, the resident will act as the attending physician for the:<\/p>\n<ul>\n<li>inpatient units,<\/li>\n<li>outpatient clinic,<\/li>\n<li>Emergency Department, or<\/li>\n<li>consult-liaison service.<\/li>\n<\/ul>\n<p>In this capacity, the PGY-4 will lead treatment team, provide guidance to younger residents, teach medical students, and make final medical decision for patients. There will always be an attending physician available for advice and recommendations, but this experience allows the PGY-4 to fully utilize the training, knowledge, and leadership skills that have been cultivated throughout residency.<\/p>\n<h3>Elective<\/h3>\n<p>The remainder of the fourth-year is an elective of the resident&#8217;s choice with a myriad of opportunities available. Faculty mentors are available as advisors to assist residents in pursuing their educational goals.\u00a0Some residents elect to pursue senior administrative responsibilities as Chief Residents, while others continue to develop their skills in clinical or research arenas.<\/p>\n<table class=\"table table-hover\" style=\"text-align: center\" border=\"0\">\n<tbody>\n<tr>\n<td style=\"background-color: #f0f0f0\" colspan=\"2\"><strong>Sample PGY-4 Electives<\/strong><\/td>\n<\/tr>\n<tr>\n<td>Mental Health Policy<\/td>\n<td>Palliative Care<\/td>\n<\/tr>\n<tr>\n<td>Memory Disorders<\/td>\n<td>Global Psychiatry<\/td>\n<\/tr>\n<tr>\n<td>Campus Health<\/td>\n<td>Clinical or Basic Science Research<\/td>\n<\/tr>\n<tr>\n<td>Psychotherapy \/ Supervision<\/td>\n<td>ECT Certification<\/td>\n<\/tr>\n<tr>\n<td>Geriatric Psychiatry<\/td>\n<td>Neuropsychiatry<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<h2>Call Schedules<\/h2>\n<p>Senior residents\u00a0continue to take 6 &#8211; 7 weekend rounding calls annually. PGY-4s see patients in the\u00a0outpatient clinic, inpatient units,\u00a0and spend 1:1 supervision time with an attending physician.<\/p>\n<h2>Moonlighting<\/h2>\n<p>PGY-4 residents may elect to supplement their clinical training with moonlighting shifts at the UNC Emergency Department (PES), Youth Behavioral Hospital (YBH), and Central Regional Hospital (CRH). While the General Residency clinical experience is very robust, senior moonlighters enjoy the ability to practice attending-level autonomy.<\/p>\n<h2>Fellowships<\/h2>\n<p>Residents who matched with the\u00a0<a class=\"external-link\" title=\"\" href=\"http:\/\/www.med.unc.edu\/psych\/education\/fellowships\/child-adolescent-residency\" target=\"_blank\" rel=\"noopener noreferrer\">Child and Adolescent Psychiatry<\/a>\u00a0fellowship begin that specialization as a PGY-4. Residents who are interested in applying for the\u00a0<a href=\"http:\/\/www.med.unc.edu\/psych\/education\/fellowships\/consult-psychiatry-fellowship-training-program\">Consultation-Liaison Psychiatry<\/a>\u00a0fellowship,\u00a0<a class=\"external-link\" title=\"\" href=\"http:\/\/www.med.unc.edu\/psych\/forensic\/fellowship-program\" target=\"_blank\" rel=\"noopener noreferrer\">Forensic Psychiatry<\/a>\u00a0fellowship, or\u00a0<a href=\"http:\/\/www.med.unc.edu\/psych\/education\/fellowships\">other<\/a>\u00a0fellowship opportunities are encouraged to submit applications during the fall of PGY-4.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The final year of residency (PGY-4) concludes training in the General Psychiatry Residency Program and allows the resident to hone skills and pursue further educational endeavors.
