Training Verification
Requests for Residency and Fellowship training verification may be submitted to the Department of Radiology Education Team. Please note that this is a separate process from a request for a professional reference, which should be directed to the specific faculty member and is voluntary.
Submit a Request
Follow the link and corresponding instructions for requesting program verification. Please be sure to provide the verification form you need completed and the signed trainee consent form as part of your request. Those forms can be emailed to RadEduTeam@med.unc.edu once you have completed the registration.
Verification Request Link- Payment: Requests submitted more than two years after a trainee has completed or left a UNC Radiology training program are subject to a $100 processing fee. There is no fee for trainees who have completed the program within the last two years.
- Limitations Note: If a trainee’s program completion date was more than five years ago, we cannot provide information related to competency, performance, or ethics. In these cases, we can only verify program participation, training dates, and program completion status.
