SUMMARY:Walk-In Clinic @ Urban Ministries of Durham
DESCRIPTION:Our monthly walk-in clinics at St. Thomas More Church (STM) and Urban Ministries of Durham (UMD) always offer blood sugar and blood pressure screenings\, medication reconciliation\, lifestyle modification counseling\, and medication questions. We also offer a preventative health consultation service\, in which participants receive personalized plans with recommendations from the US Preventive Services Task Force and CDC Immunization Schedule in order to stay healthy for the future. Additional services are offered at some walk-in clinics\, and may include physical therapy as well as other evaluations. Additional services are offered at some walk-in clinics\, and may include physical therapy evaluations. \nAt both STM and UMD\, we have established partnerships with many local clinics including SHAC Clinic\, Samaritan Health\, and Lincoln Health for patients needing follow-up care from our screenings. We also have a list of resources for patients seeking any form of medical care\, and we will work to put you in touch with a local clinic or program that meets your needs. \nWe always offer Spanish interpretation services\, especially at our Saint Thomas (STM) location.
