Our Mission<\/h3>\n<\/div>\n

The \u201cBridge to Care\u201d Clinic provides continuing medical care to patients with chronic conditions such as diabetes, hypertension, hyper\/hypothyroidism, etc. Medical visits are provided during regular clinic hours on Wednesday evenings. Our ultimate goal is to bridge patients to more long-term healthcare. We are also able to connect patients to other resources within SHAC such as SHAC Wellness, the Medication Assistance Program (MAP), SHAC Dental, etc.<\/p>\n<\/div>\n<\/div>\n

About Us<\/span><\/h2>\n

As a part of Bridge To Care, you will meet with medical professionals who specialize in chronic care. We can also refer patients to a local provider and, if necessary, apply for charity care. SHAC has relationships with other community resources and can help refer you to free\/reduced priced clinics that will also help you with your conditions.<\/p>\n
