“Transforming patient generated health data into value for patients, teams, and care delivery”<\/p>\n

This talk explores the power of patient generated health data and state-of-the-art in applied measurement of patient reported outcomes (PROs), patient experience, and patient engagement. Together we’ll journey through the use of these tools to prepare (build relationships among patients and teams), perform (enable shared decision making), and practice (drive quality improvement and continual learning) in the transformation toward high value care.<\/p>\n
