The Rural Health Transformation Program is developing a plan to improve health and healthcare across our communities over the next five years—and we need your input.
Join NC ROOTS Region 4 for a community engagement session where we will share findings from our Regional Needs Assessment and hear directly from community members, local organizations, healthcare providers, advocates, and other partners about what matters most.
During this session, participants will:
- Review key findings from our assessments of strengths and needs in NC Roots Region 4
- Discuss the health challenges and barriers facing local communities
- Identify priorities for future investment and action
- Help shape a Regional Action Plan that will guide rural health improvement efforts and investments over the next five years
Everyone who joins will receive a gift card in thanks for their participation in our NC ROOTS Region 4 planning! We want to hear from community members from the following Region 4 counties: Alamance, Caswell, Chatham, Durham, Franklin, Granville, Johnston, Nash, Orange, Person, Vance, Wake, Warren, and Wilson counties.
