Dear families we serve,<\/strong><\/p>\n

As part of the terrific Diagnostic and Complex Care Clinic team, and since 2006, our mission is to serve children, adolescents and young adults with chronic conditions and their families. Our interdisciplinary team at the S<\/u><\/strong>elf-management and T<\/strong>ransition to A<\/u>dulthood with Rx<\/sub><\/u><\/strong>=treatment (STARx<\/sub><\/strong>) Program has continued to provide new evidence in the field of healthcare transition from pediatric- to adult-focused health services. The STARx team has published over 80 scientific manuscripts in this field with new collaborators from around the country and the world. Our tools (STARx<\/sub> Questionnaire, TRx<\/sub>ANSITION Index and STARx \u00a0<\/sub>Passport) have been translated into 8 languages and are being used in 10 countries.<\/p>\n

We are grateful to Dr. Carl Seashore as his persistence made it possible for the self-administered STARx<\/sub> Questionnaire to be now in EPIC, our electronic health record. Our collaborators at the Children\u2019s Hospital of Orange County have our tools in the electronic health record called CERNER. Our patient education tool \u201cALL YOU NEED IS LOVE Syllabus\u201d is being validated and evidence of its effectiveness among USA and Mexico patients has been published in peer-reviewed journals. In collaboration with the 3C Institute, the Kidney Guide App has been created and will be deployed in several institutions across the USA.<\/p>\n

I am also grateful for the accomplishments the International and Interdisciplinary Healthcare Transition Research Consortium has made since I co-founded it in 2007. And last, but not least, I am grateful for the work we have accomplished with Dr. Peter Kotanko and his team at the Renal Research Institute by hosting the annual International Pediatric Nephrology Fellows Conference and the PICCOLO MONDO Initiative. \u00a0 \u00a0<\/strong><\/p>\n

Como parte del excelente equipo de la Cl\u00ednica de Diagn\u00f3stico y Atenci\u00f3n a Pacientes con Condiciones Complejas, y desde 2006, nuestra misi\u00f3n es atender a ni\u00f1os, adolescentes y adultos j\u00f3venes con enfermedades cr\u00f3nicas y sus familias. Nuestro equipo interdisciplinario del Programa de Automanejo y Transici\u00f3n con Rx=tratamiento (STARx) ha seguido brindando nueva evidencia en el campo de servicios de salud pedi\u00e1tricos a servicios de salud enfocados en adultos. El equipo STARx ha publicado m\u00e1s de 80 manuscritos cient\u00edficos en esta \u00e1rea con nuevos colaboradores en EE. UU. y el mundo. Nuestras herramientas (Cuestionario STARx, \u00cdndice TRxANSITION y Pasaporte STARx) se han traducido a 8 idiomas y se utilizan en 10 pa\u00edses.Agradecemos al Dr. Carl Seashore ya que su persistencia hizo posible que el Cuestionario STARx autoadministrado est\u00e9 ahora en EPIC, nuestro registro de salud electr\u00f3nico. Nuestros colaboradores del Children\u2019s Hospital of Orange County cuentan con nuestras herramientas en la historia cl\u00ednica electr\u00f3nica denominada CERNER. Purpose<\/span><\/h2>\n

Founded in 2006, the main focus of the STARx Program is to develop and improve self-management skills and disease knowledge among adolescents and young adults with chronic health conditions. We have created the TRxANSITION Passport, TRxANSITION Index (provider verified), and the self-administered STARx Questionnaire, self-management and transition readiness assessment tools <\/a>to identify areas of knowledge and skill deficits among patients ages 12 and older. Based on identified areas of need, education and interventions are then provided in \u00a0an individualized, culturally, linguistically and age-appropriate fashion.<\/p>\n
