The TRxANSITION Index\u2122<\/h2>\n<p>The <a class=\"internal-link\" title=\"\" href=\"https:\/\/www.med.unc.edu\/transition\/transition-tools\/starx-questionnaire\" target=\"_self\" rel=\"noopener\">TRxANSITION <\/a>Index is a 10-domain, 32-item questionnaire measuring transitions readiness that is administered by a trained professional. Administering the instrument takes about 7-8 minutes and can be done either in person or over the phone. The <a class=\"internal-link\" title=\"\" href=\"https:\/\/www.med.unc.edu\/transition\/transition-tools\/copy_of_educational-handouts-for-trxansition-indextm\" target=\"_self\" rel=\"noopener\">TRxANSITION Index\u2122 Educational Handouts<\/a> provide patient education that corresponds with each subdomain of the instrument.<\/p>\n<h2>2. STARx Questionnaire<\/h2>\n<p>The <a class=\"internal-link\" title=\"\" href=\"https:\/\/www.med.unc.edu\/transition\/transition-tools\/trxansition-scale\" target=\"_self\" rel=\"noopener\">STARx Questionnaire<\/a> is a 3-domain, 18-item self-administered questionnaire measuring overall transition readiness. Patients can complete the STARx Questionnaire in 3 minutes on paper or on the computer.<\/p>\n<h2>3. Transition Passport\u2122<\/h2>\n<p>The <a href=\"https:\/\/www.med.unc.edu\/transition\/transition-tools\/trxansition-medical-passport\">Transition\u00a0Passport\u2122<\/a> is a portable health summary the size of a driver&#8217;s license that a patient can carry with them in their wallet. The passport can be used for patient education as well as a tool for communication between patient and provider.<\/p>\n<h2>4. ALL YOU NEED IS LOVE TRxANSITION syllabus<\/h2>\n<p><span class=\"description\"><a href=\"https:\/\/www.med.unc.edu\/transition\/current-projects\/all-you-need-is-love-adherence-and-longitudinal-life-skills-for-youth-under-a-nurturing-educational-environment-on-disease-intelligent-self-management-lasting-outcomes-visionary-empowerment\">ALL YOU NEED IS LOVE<\/a> (Adherence and Longitudinal Life skills for Youth, Under a Nurturing Educational Environment on Disease-Intelligent Self-management: Lasting Outcomes, Visionary Empowerment) is a syllabus and educational tool for patients involving the development of a self-education module for promoting self-management, adherence, and mindfulness among adolescents and young adults with chronic kidney disease (CKD). <\/span><\/p>\n<p>&nbsp;<\/p>\n<p>View our web-based <a href=\"https:\/\/domserv.med.unc.edu\/Starx\/transx_files\/questionaire\/\">data entry system<\/a>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>1.
