Administering the instrument takes about 8 minutes and can be done either in person or over the phone. The TRxANSITION Index can be administered through our web-based data entry system (requiring a <a class=\"internal-link\" title=\"\" href=\"https:\/\/www.med.unc.edu\/transition\/wp-content\/uploads\/sites\/523\/2017\/12\/Guest-ONYEN-Form.docx\" target=\"_self\" rel=\"noopener\">guest ONYEN<\/a>) or with a hard copy of the instrument.<\/p>\n<h2>Scoring the TRxANSITION Index<\/h2>\n<p class=\" \">Each item is scored individually as either 1 (adequate knowledge\/skill mastery), 0.5 (some knowledge\/skill attainment), or 0 (no knowledge\/skill attainment). Subscale scores are calculated by dividing the patient&#8217;s score by the total possible subscale score. Subscale scores are then combined to create a total score, ranging from 0 to 10. Individuals administering the TRxANSITION Index should familiarize themselves with the <a class=\"internal-link\" title=\"\" href=\"https:\/\/www.med.unc.edu\/transition\/wp-content\/uploads\/sites\/523\/2017\/12\/TRxANSITIONScaleAnswerGuideforKidneyPatients.pdf\" target=\"_self\" rel=\"noopener\">scoring guide<\/a>.<\/p>\n<h2 class=\" \">Using the TRxANSITION Index to Inform Care<\/h2>\n<p>The TRxANSITION Index has different <a class=\"internal-link\" title=\"TRxANSITION Scale Areas of Concentration by Age\" href=\"https:\/\/www.med.unc.edu\/transition\/wp-content\/uploads\/sites\/523\/2017\/12\/TRxANSITIONScaleAreasofConcentrationbyAge.pdf\" target=\"_self\" rel=\"noopener\">areas of concentration by age<\/a>.
