The questionnaire contains three subdomains that focus on three different areas of transition related knowledge &#8211; communication with medical provider, disease knowledge, and self-management. There are <strong>three versions<\/strong>, depending on the setting. <strong>1<\/strong>.\u00a0<a href=\"https:\/\/www.med.unc.edu\/transition\/wp-content\/uploads\/sites\/523\/2024\/08\/STARx-Questionnaire_Patients-in-Pediatric-settings.doc\">STARx Questionnaire Patients in Pediatric settings<\/a>, <strong>2<\/strong>\u00a0<a href=\"https:\/\/www.med.unc.edu\/transition\/wp-content\/uploads\/sites\/523\/2024\/08\/STARx-Questionnaire_Patients-in-Adult-focused-settings.doc-20-52-49-101.doc\">STARx Questionnaire Patients in Adult-focused settings<\/a> and <strong>3<\/strong>\u00a0<a href=\"https:\/\/www.med.unc.edu\/transition\/wp-content\/uploads\/sites\/523\/2018\/04\/STARx-Questionnaire_Parent-version.doc\">STARx Questionnaire Parent version<\/a><\/p>\n<h2>Administering the STARx Questionnaire<\/h2>\n<p>The <a href=\"https:\/\/www.med.unc.edu\/transition\/wp-content\/uploads\/sites\/523\/2018\/04\/STARx-Questionnaire_Patients-in-Pediatric-settings.doc\">STARx Questionnaire Patients in Pediatric settings<\/a> is a 3-domain, 18-item self-administered questionnaire. Patients can complete the STARx Questionnaire in 3 minutes on paper or on the computer. The STARx Questionnaire can be administered through our <a class=\"external-link\" title=\"STARx Online Database\" href=\"https:\/\/domserv.med.unc.edu\/Starx\/transx_files\/questionaire\/\" target=\"_self\" rel=\"noopener\">web-based data entry system<\/a> (requiring a <a class=\"internal-link\" title=\"\" href=\"https:\/\/www.med.unc.edu\/transition\/wp-content\/uploads\/sites\/523\/2017\/12\/Guest-ONYEN-Form.docx\" target=\"_self\" rel=\"noopener\">guest ONYEN<\/a>) or with a hard copy of the instrument.<\/p>\n<h2>Scoring the STARx Questionnaire<\/h2>\n<p>Each item is scored individually and worth a maximum of 5 points. Each item is answered by the patient on a 1 &#8211; 5 Likert scale and receive a corresponding score of 1 &#8211; 5 points for each item. Subdomain scores are combined to create a total score, ranging from 0 &#8211; 90. Individuals administering the STARx Questionnaire should familiarize themselves with the <a class=\"internal-link\" title=\"STARx Scoring Guide\" href=\"https:\/\/www.med.unc.edu\/transition\/wp-content\/uploads\/sites\/523\/2017\/12\/starx-scoring-guide.doc\" target=\"_self\" rel=\"noopener\">STARx Scoring Guide<\/a>.<\/p>\n<h2 class=\" \">Using the STARx Questionnaire to Inform Care<\/h2>\n<p>The STARx Questionnaire is a brief instrument that can be used during appointments to check in with adolescents and young adults. Using our <a class=\"external-link\" title=\"STARx Online Database\" href=\"https:\/\/domserv.med.unc.edu\/Starx\/transx_files\/questionaire\/\" target=\"_self\" rel=\"noopener\">web-based data entry system<\/a>, reports can be printed immediately after the patient completes the survey.
