Participation Requirements

I am a Primary Care Improvement Collaborative (PCIC) participating physician or physician assistant who is actively involved with at least one of the following:

  • Provide direct or consultative patient care
  • Oversee clinical delivery activities that provide opportunities for improvement
  • Serve as a QI lead, coach, or a resident/student advisor, or other leadership role

I participated in meeting with others involved with the QI Project including:

  • Reviewed performance data not less than 1 time, including at baseline and prior to the completion of the activity for MOC purposes.
  • Helped to develop and/or apply tools and interventions.
  • Reflected on the impact of the initiative on the practice and/or organization.

I verify and attest that I have: 

  • Identified and/or acknowledged a gap(s) in outcomes or in care delivery 
  • Identified and/or reviewed data related to the gap(s) 
  • Identified or acknowledged appropriate intervention(s) designed to improve the gap(s), OR participated in the planning and selection of intervention(s) designed to improve the gap(s) 
  • Implemented intervention(s) for a timeframe appropriate to addressing the gap(s), OR monitor and manage implementation of intervention(s) for a timeframe appropriate to addressing the gap(s). 
  • Reviewed data related to the gap(s). 
  • Reflected on outcomes to determine whether the intervention(s) resulted in improvement. If no improvement occurs after an intervention, reflected on why no improvement occurred.
