Program Fidelity Documents

The forms and documents below are used by early psychosis programs in North Carolina. They are available for download with acknowledgement of the statement below. The purpose of these documents is to assist the evaluation of service delivery and program fidelity. They are designed to be integrated into clinical care and to inform treatment by capturing key aspects of recovery from the perspective of patients, providers, and the community. These documents include data collection forms, program fidelity guidelines and evaluation, and an example report. Copies of the REDCap Quality Improvement database and a template for report creation available upon request.
