How to Request Clinic Services
Individuals, families, or referring providers may submit a request for clinic services by completing our CIDD Referral Form. Once submitted, your request will be added to our waitlist. Please note there may be extended wait times (possibly several months) before you are contacted to arrange an appointment. Our scheduler will contact the individual or family to arrange a visit once they reach the top of the waitlist and an appointment becomes available with the provider(s) best suited to address referral needs.
If you are interested in services through our Angelman Syndrome Clinic or the Dup15 Syndrome Clinic, please do not submit a CIDD Referral Form. Instead, please complete our online Angelman/Dup15 Intake Form. Please note we are not currently accepting referrals online for our other clinics.
If you are a school program or other agency wishing to contract services through the CIDD, please visit our Contracting for Services page.
