Heart Health Now: Implementing Risk Stratification, Data Tools, and QI Coaching Across NC
In 2015, Dr. Cykert — in partnership with Healthcare Research and Quality, Community Care of North Carolina, and North Carolina Area Health Education Centers — developed Heart Health Now, a project working toward combating cardiovascular disease in North Carolina.
Dr. Cykert will discuss efforts from the past 4 years to reduce cardiovascular risk for over 700,000 adult patients in 219 practices across the state. A recent podcast with UNC’s “Well Said” provides more detail.
