Patient and Family Advisory Council (PFAC)

What is a PFAC?

• A PFAC helps improve the patient experience and enhances the quality of care
• It includes patients and family members of patients
• You’ll discuss your personal experiences and viewpoints to help staff develop and evaluate programs and services

Am I Eligible?

To be in the PFAC you must be:

• 18 years old or older
• Be a patient in the ACHD Program

                                   Or

• Be the family/caregiver of an ACHD patient
• Have MyChart and internet access
• Be available for meetings every 3 months

How to apply
Ask our ACHD Nurse Coordinator for a paper application
You will be contacted for a virtual interview
