April Goley is the Lead APP of the Diabetes Care Team at UNC Hospitals in Chapel Hill. Our team mission is to improve outcomes of patients admitted at UNC through better glycemic control. To achieve our mission we work closely with other departments to promptly and effectively conduct an inpatient consult on any patient with an abnormal glucose value. Through this critical early involvement and close monitoring throughout one’s hospitalization, we are able to make a significant impact. To further aid in the transition back to outpatient environment, we conduct virtual visits after discharge for patients who may need revisions in their regimen before they are able to see their outpatient provider.

In addition to providing direct patient care through consults and follow-up visits in the hospital and close after discharge, the Diabetes Care Team assist with broad educational efforts, involved in clinical research and quality improvement initiatives.
