I manage heart failure patients throughout the spectrum from patients with a new diagnosis to those needing advanced heart failure therapies. I see patients in clinic for hospital follow-up, medication optimization, and urgent visits. I also follow patients through remote heart failure monitoring (CardioMems and thoracic impedence).
