Stephenie Fenton-Wilhelm and Dr. Bhisham Chera presented, “Using Good Catches to Improve Quality and Safety” at IHQI’s Speaker Series event, held February 11, 2020 in Bondurant G-100.

Stephenie Fenton-Wilhelm, Vice President of Risk Management for UNC Health Care, spoke first. She explained the changes in the patient safety reporting framework at UNC Medical Center that have occurred since 2017. Additionally, she illustrated the flow of information in the quality and safety reporting system, stressing the importance of a feedback loop. Finally, she highlighted successful improvement initiatives undertaken as a result of patient safety reporting.

Dr. Bhisham Chera, Associate Professor; Chair of Clinical Operations and Improvement; Director of Patient Safety and Quality in the Department of Radiation Oncology, followed, detailing Radiation Oncology’s incident learning system. He outlined his department’s approach to analyzing and celebrating “good catches,” providing concrete examples and emphasizing the importance of ongoing feedback, strong commitment from leadership and a desire to make incident learning “a part of what we do.”

A robust question and answer period concluded the program. Feedback from the event included comments such as, “This was awesome! Speakers were very knowledgeable and provided great examples.”
