The Geriatrics Specialty Clinic (GSC) is part of The University of North Carolina Hospitals and assists older individuals with complex medical, social and functional problems. Geriatricians within this clinic provide comprehensive care with a focus upon quality of life, family support and helping patients live as well as they can for as long as they can. During the initial visit, patients and their families are evaluated by a geriatric physician who can provide either consultative or ongoing primary care.
Patients cared for in the GSC may be faring well with stable medical problems yet wish to establish a relationship with a physician with a focus upon maintaining their health. Other patients may be suffering from chronic and debilitating illness such that they and their families need a physician to coordinate care of ongoing medical problems and to provide guidance as they transition from one level of care to another.
Clinical issues often addressed at the GSC include the following:
|Confusion and Memory Difficulties
|Frailty with Functional Decline|
|Depression||Advance Care Planning|
|Imbalance and Falls||Medication Management|
|Urinary Incontinence||Other chronic and debilitating medical problems|
