For New Patients
Welcome! We are eager to meet you and begin your journey to better hearing. Prior to your first appointment, you will be sent a case history form and service authorization form (by email or mail). Have your documents completed prior to your appointment. Please arrive 15 minutes before your appointment for paperwork and processing. If you have not received your new patient forms, contact our front desk at: (919) 445-6744.
What to Bring to Your Appointment
- health history form and signed service authorization
- primary care provider’s name, address and phone number
- any required referrals from your primary care physician
- insurance card and any other related forms required by your insurance company
- list of current medications

Parking
For patients arriving to our Chapel Hill office, parking spaces marked ‘RESERVED FOR 6015’ are reserved for you! You are welcome to park in those or any available space in the parking lot. For patients arriving to our Hillsborough office, you are welcome to park wherever is convenient.
