Patient Safety and Reporting Adverse Events
Fundamentally, patient safety refers to freedom from accidental or preventable injuries produced by medical care. Practices or interventions that improve patient safety are those that reduce the occurrence of preventable adverse events. This can be consistently achieved in an organization with a strong safety culture.
To report a Safety Issue, use the Patient Occurrence Reporting System (PORS) icon on the intranet. There is also an option to log in as ‘anonymous.’
The Brigham and Women’s Hospital have created several videos with examples of disclosing an adverse event. IHQI also provides Sorry Works!
