Patient Safety and Reporting Adverse Events
Safety is one of the six aims of quality. In order to promote patient safety, health care professionals need to experience psychological safety themselves. A just culture – one that recognizes that people work in systems, and that system failures are to blame for errors rather than individuals – is essential to supporting greater reliability within organizations.
UNC Health has the SAFE reporting system to identify errors in patient care. All employees are encouraged to submit SAFE reports for errors and near misses. Reports can be submitted anonymously.
