Referring a Patient

1.  Provide Referral

For UNC Providers or those with UNC Carelink

• Click ““Ambulatory Referral to UNC Burn Center” or use Code: REF255
• In “To Dept” choose “UNCH Multispecialty Surgery Burn Chapel Hill”
• In order to expedite the care of your patient, please provide the specific reason for the consult request and fully complete the referral form. Please include the patient’s current contact and insurance information.
• To obtain a CareLink Account, click on UNC CareLink and select “Request an Account.”

Or send via Fax

• To bypass UNC CareLink, please fax a completed referral form (link pending), including the specific referral question and pertinent records to (phone number pending).   If you have any questions, please call 984-974-1050.

2. Provide test results prior to the first appointment

• Test results should be provided (via Powershare, fax, or mailed prior to the first appointment.  This allows providers to review all information prior to seeing the patient.
